|half day sessions in outpatient gastroenterology and pancreas clinic seeing primary as well as second opinion |

|consults, and seven half day sessions performing endoscopy (diagnostic and therapeutic upper endoscopy, |

|colonoscopy, enteroscopy,) In addition, I attend on the inpatient gastroenterology consultation service for six |

|weeks annually. On an average I perform 45-50 endoscopies and see 40 consults per week. |

|I am the Co Director of the multidisciplinary Pancreas Center at the Beth Israel Deaconess Medical Center. I see patients for a second opinion, from within|

|Massachusetts and well as out of state with an established diagnosis of pancreatic disease as well as patients with unexplained chronic abdominal pain. I |

|personally take care of more than 200  patients with chronic pancreatitis, or those with abdominal pain thought to be from chronic pancreatitis, recurrent |

|acute pancreatitis, autoimmune pancreatitis, pancreatitis associated with mutations of cystic fibrosis gene and patients with cystic neoplasms of the |

|pancreas. In addition to providing consultative service in the Pancreas Center, I am actively involved in several clinical and translational research |

|projects including examining the role of MR secretin pancreatography and endoscopic pancreatic secretin stimulation testing in patients with chronic |

|pancreatitis and unexplained abdominal pain. Our research has shown that endoscopic pancreatic secretin stimulation is a very sensitive test in detecting |

|early chronic pancreatitis in patients with normal radiologic and endoscopic testing. Approximately forty percent of patients with normal imaging studies |

|and suspected chronic pancreatitis who have abnormal secretin tests, when followed prospectively, developed imaging abnormalities at a median of two year |

|follow up. |

|I am an active member of our world renowned celiac center and follow more than 400 patients with an established diagnosis of celiac disease. As a celiac |

|center clinician I see patients from all over New England and from other parts of the country. I am a co-investigator in several research projects |

|including treatment of refractory celiac disease with mesalamine, disorders mimicking celiac disease, HLA testing in celiac disease and the impact of |

|socioeconomic status on management of celiac disease. |

| |

|My research on cystic fibrosis (CF) gene mutations and liver disease established a novel link to primary sclerosing cholangitis (PSC). We showed that a |

|very high proportion of patients with inflammatory bowel disease and PSC had disease causing CF mutations. These findings lead to a collaborative study |

|with the Mayo Clinic to examine the role of docosahexaenoic acid (DHA) in the treatment of PSC. This was also based on the fact that our prior research |

|had shown that CF homozygous and heterozygous mice with chemical colitis developed changes mimicking PSC. Some of these changes could be reversed by |

|administration of DHA. |

|I have an active interest and advanced training in endoscopy. I am also the lead teacher in endoscopic techniques for fellows. Active research in advanced |

|endoscopy and quality improvement includes projects examining adenoma detection rates at index colonoscopy as well as predicting polyp histology non |

|invasively at colonoscopy using narrow band imaging. |

|I am one of the leading clinician-educators of our Gastroenterology Division and play a pivotal role in training GI fellows. For the past 8 years I have |

|a) trained our second year GI fellows in endoscopic procedures one day per week and b) have been a primary mentor to the third year GI fellows in out |

|patient clinics as well as endoscopy sessions 3 days each week. During these sessions I have provided ongoing training in general Gastroenterology and |

|Hepatology consultations as well as advanced training in pancreatology and in celiac disease. I also provide our GI trainees with advanced training in |

|therapeutic endoscopy including stricture dilatation, removal of large polyps, endoscopic mucosal resection, control of active bleeding with new hemostasis|

|techniques and placement of wireless pH probes. I also play a major role in teaching Harvard medical students and rotating medical residents on a regular |

|basis on the GI consult service and in the out patient clinics. |

|In summary, I am an established and well recognized clinician, educator and clinical investigator in the Division of Gastroenterology at the Beth Israel |

|Deaconess Medical Center. My special areas of focus and achievement lie in my GI clinical expertise and acumen, in education in general gastroenterology, |

|and in my advanced expertise in both pancreatology and celiac disease. I am proud and happy to act as a lead educator of future gastroenterology fellows |

|in both GI consultation and endoscopy skills. I am also actively and enthusiastically involved in translational research in pancreatic and celiac disease |

|research in collaboration with my colleagues at BIDMC and beyond. |
