CCSD Pre-K (CD) Application Procedures for School Level 2020

Pre-K Intake form for the Application/Screening Process.

The intake form is for Parents/Guardians of a 4 year old (by September 1, 2020) with intent to apply and be screened.

The Pre-K intake form will be uploaded to CCSD website beginning January 28, 2020. Paper copies of the intake form will be sent to schools and will be available in English and Spanish. Families can access the intake form from their zoned school and located at . The paper intake form, once complete, will be sent through courier to Pam Waters, Early Learning Department.

Parents complete the intake form one of two ways: paper copy or online at . Encourage families to take their time to complete the entire intake form. If the parent is filling out by hand, ask them to Please Print (do not use cursive writing) and review carefully all responses to verify all information is complete, legible, and accurate.

2. Parents will be contacted by the Early Learning Department to select a date and time for child screening in April (Round 1). DIAL-4 screening will be completed by a centralized screening team. (Round 2 & 3 Screenings will be scheduled in June).
