Your choice of a provider is your own. We will help you make your choice by providing details on providers' credentials, type of services they offer, and their performance on quality measures and surveys. This information is not an endorsement of a particular provider's suitability for your needs.

You will normally have lower out-of-pocket costs by visiting a provider in the network. These providers have agreed to provide financial discounts and participate in programs to help you have a good member experience.

Important Notice:

Some network providers may have been added or removed from our network after this directory was printed. Soon after a provider changes their status, out of network penalties or increased cost sharing will result. In some cases, extension of in network or greater network benefits may be available. We do not guarantee that each provider is still accepting new patients. Any physician included in this directory is listed for outpatient office visits. In addition, the directory includes information regarding whether the provider is currently accepting new patients. You can find the most up-todate information about network providers by using our online directory at .

The providers in this directory may not necessarily reflect the full extent of the network of contracted providers. There may be providers or certain specialties that are not included in this application that are part of our network. If you don't find the provider you are searching for, you may contact the provider directly to verify participation status with the network, or contact Customer Care at the toll-free number shown on your ID card. We also recommend that, prior to seeing any provider, including any specialists and behavioral health providers, that you call the provider's office to verify their participation status and availability, since a provider's availability to see new patients may change frequently. The information included in the directory is accurate, to the best of the carrier's knowledge, as of the date of updating.
