To: Parents/Guardians of Students Interested in Participating in School Athletic Teams.

All students wishing to try out for a school athletic team must undergo a physical examination within 365 days prior to the first practice session of that team. No student will be allowed to try-out or participate in a school sport unless the following sports forms are completed and the student’s forms have been reviewed by the school physician and granted approval to participate in any school athletics.

INSTRUCTIONS FOR COMPLETING SPORTS FORMS

** New Forms were provided by the NJ Department of Education in May 2014****

STEP 1: Obtain appropriate forms: New Jersey Department of Education’s Pre- Participation Physical Evaluation Form--- HISTORY FORM & PHYSICAL EXAMINATION FORM. (These forms are available at the Main Office, Health Office, or on-line at the school’s website: ).

STEP 2: Parent must first complete the HISTORY FORM. Answer every question. Do not leave any question blank or this form will be returned to you for completion which may cause a delay in your child’s sports participation.

STEP 3: Bring BOTH parts to your child’s healthcare provider. The provider performing the physical exam on your child must review the HISTORY FORM completed by the parent, at the time of the physical examination. Before you leave the provider’s office make sure the provider has completed the form in its entirety as any blanks will be returned to you for completion.

STEP 4: Bring all completed forms to the Health Office by the deadline date. The school physician will come to the school on a set date before the start of each season to review the forms. The physician will reject any form not completed correctly. This may cause a delay in your child’s sports participation so please make sure all forms are completed.

HEALTH HISTORY UPDATE QUESTIONNAIRE: PLEASE ALSO NOTE, if your child’s medical examination was completed more than 90 days prior to the sport’s first day of official practice, a parent must also complete a Health History Update Questionnaire. Any changes in health status may require a note from your physician stating “Cleared for Sports”.
