Submitting immunization documents through myWingsIf you have a computer or laptop and a scanner, you can upload all necessary documents through myWings. Instructions are at the top right of the UNF Immunization Form.Note:Tablets and smart phones are not recommended for successful data transmission.Recommended browsers are Google Chrome, Firefox, and Internet Explorer.FREE document scanners available for download are CamScanner, FineScanner, and Scanbot.Required documents1. Your Immunization/Vaccine or IgG Titer Documents It is not necessary to have a physician or nurse sign and stamp the UNF form if your records: Contain your name and date of birth; all vaccines and dates; and your physician/clinic, school, military location (DD 2766) or employee health record pre-printed or ink-stamped on the document orAre from an electronic health portal databaseAre a positive Measles/Rubella IgG titer lab result reportFAQsQ: How do I submit the necessary documents if I do not have a printer?A: The UNF Immunization Form is a fillable PDF file. If you get an error on “filling of form,” try a different browser, as not all browsers support fillable documents. After you download the form, you can click each blue-grey box and type in your information. Do not leave blank the boxes that require a signature; type your name again. Save the document and follow instructions at top right of the form.Q: What if I don’t have my records? A: Complete the form on the left side and sign and date the waiver of liability statements on the right side (boxes #2 and #3). Submit your form per instructions at the top right. Options to complete the submission of your vaccine records:Contact your doctor’s office and have them fax a copy of your records to Medical Compliance at 904-620-2901 or 904-620-2902. If a release form is required in order to send your records to us, please complete “Medical Release - Part 2,” found on the Medical Compliance website under Forms and Documents. If you can’t get the completed form to the doctor’s office, email it to us at medical_compliance@unf.edu and we will fax it to them.If you had immunizations in Florida, you may be able to access your records directly through the state database, Florida SHOTS (information below). If you had immunizations in another state, check with your local health department for electronic access to the state database or health portal.We may be able to retrieve your records if you are an incoming freshman, your high school transcripts include your immunizations, and your transcripts were sent electronically to UNF. Additionally, you may be able to download it directly from your own computer. Ask your health care provider for the State IMM Id and Certification PIN of your child's DH Form 680.?
