Carroll County Flu Vaccination Resource Sheet

Getting a flu vaccine every year is the best way to protect yourself and your family from illness, missed work and school, and serious health problems related to the flu. CDC recommends that everyone 6 months and older get vaccinated. To find a flu vaccination, visit:

Your Provider's Office: Ask your primary care or pediatrician's office about getting the flu vaccination. Most offices have it available for their patients. Ask if your insurance will cover the vaccine and if there is a copay.

Your child's school: Currently the Health Department, Carroll County Public Schools, and the Maryland Partnership for

Prevention have scheduled no-cost flu shot clinics for CCPS students only at county public schools on the dates below. A clinic for children who do not attend Carroll County Public Schools will be scheduled for November. Watch this page and our social media for details.

For Carroll County Public Schools students who wish to get their flu shot at their school clinic:

Register your child BEFORE Tuesday, October 19:

Fill out the consent form online at:

1. Do NOT check "flu" or "school" box 2. Type your child's school name in Search by Name then press the blue Search 3. Click on the Sign Up for Childhood Vaccinations button for your child's school.

OR

Fill out the Carroll County Public Schools Flu Vaccination Consent Form.

Please note:

4. Use your insurance card to fill out your insurance information accurately. Your insurance company will be billed. There is no co-pay or deductible. You will NOT be charged by us or your insurance company.

5. Read the Vaccine Information Statement (VIS) and talk to your health care provider or the school nurse about any questions you have. VIS:
