TC-4 UNEMPLOYMENT BENEFITS VERIFICATON

This form should be used when a household member 18 years of age or older reports that he/she has applied for or is receiving Unemployment Insurance Benefits. (Refer to forms TC-59a “Subsidy Interview and TC-19 “Statement of Non-Employment) The TC-4 would also be used to verify that the household member is no longer receiving benefits.

NOTE: People are usually NOT eligible for Unemployment Benefits unless they have been employed at least 6 consecutive months out of the last 12 months. (Refer to TC-18 “Termination of Employment”)

These instructions should be followed to complete the TC-4:

1. Ensure that the first half of the form is completely filled out with all pertinent information: Unemployment Office address, phone and fax; Property Address, phone and fax; household members name, Social Security Number and date, etc. NOTE: Be sure to verify the correct Social Security number before sending the TC-4.

2. The Property Manager must indicate when they wish the form to be returned. Also the Property Manager must sign where indicated.

3. The person being verified must sign and date the verification form. This shows the Unemployment Office that the household member authorizes them to release the information.

4. Make a photocopy of the signed original and place it in your pending file.

5. The original verification should be mailed or faxed to the appropriate Unemployment Office for completion. See below for exceptions to this rule.

a. If you do not receive a reply within 5 – 7 business days by mail or 2 – 3 days by fax, call the Unemployment Office and request the information again. Be sure to re-verify the mailing address and/or fax number so that a copy of the verification can be returned to them for written confirmation.

b. If the Unemployment Office will not cooperate with your request, it may be necessary to send the household member directly to the Unemployment Office for a computer printout showing the appropriate income information. If the household member returns with any other form of verification, please call your Occupancy Supervisor for Assistance.

c. If the Unemployment Office will not cooperate with the household member’s request, it may be necessary to accept photocopies of the member’s Benefit checks or check stubs as proof of income. We should obtain photocopies of 2 – 3 Benefit Checks/stubs if possible.

d. If the household member has applied for Benefits but is not receiving yet, AND the Unemployment Office won’t verify the member’s request, a copy of the Unemployment Benefits application can be used to verify their status. Property Managers will need to follow up with the household member when Benefits are finally paid.

e. If the household member’s Benefits have been terminated AND the Unemployment Office won’t verify the termination, the member will usually have correspondence from Unemployment stating this fact. This information can also be indicated on the last Benefit payment check stub. Property Managers should photocopy this information for verification.

Please call your Occupancy Supervisor for assistance if NONE of the above can be obtained.

6. When the verification is returned to you, please check it for completion. If the form is not filled out completely, comments are unclear, or amounts have been altered, you must call the Unemployment Office and verify the information in question. Be sure to use a TC-12 “Clarification Record” to record the information you are verbally given. Be sure to sign and date the form. The TC-12 should NOT be used, by itself, as a verification of income.

Remember that Unemployment Benefit income must be annualized. This is because rent should be determined based on the monthly income received at the time. When Benefits discontinue, which is usually reported by the member, a HUD Interim or USDA-RD Unscheduled certification would be completed to verify the change.

As stated above, if the Unemployment Office will not complete the TC-4 form, you can accept information as stated in paragraph 5, b – e above as appropriate to the situation. This alternative information must be attached to the TC-4 form and then placed in the applicant/resident file.
