This position provides a variety of professional clinical social work services involving; case management, completing psychosocial assessments, therapeutic interventions, individual and group therapy and discharge planning to patients with severe psychiatric disorders at Walter P. Reuther Psychiatric Hospital.This is a Drug and Alcohol-tested (DART) position under the criteria provided in the collective bargaining unit agreement due to regular contact with hospital patients.15.Please describe the assigned duties, percent of time spent performing each duty, and what is done to complete each duty.List the duties from most important to least important. The total percentage of all duties performed must equal 100 percent.Duty 1General Summary of Duty 1% of Time35Develop and plan social work services for assigned patients.Individual tasks related to the duty.Interview patients, family members and others to develop his/her psychosocial assessment.As a member of Interdisciplinary Treatment Team, assist in formulating a plan of service for each assigned patient that is guided by the person-centered planning process.Formulate plans to resolve problem/needs identified in area of expertise and evaluate progress toward treatment goals.Evaluate effectiveness of social work service plan; make revisions in plans as necessary.Integrates, coordinates, monitors and assures implementation of each patient’s individual plan of service.Develops treatment plans that are individualized, problem specific and includes:a) Participation by patient is documented (engage patient in treatment planning/ discharge planning) b) Specific problems to be treated during hospitalizationc) Physical problems that require treatment or interfere with treatment are identified and includedd) Goals are behaviorally specific and measurable e) S.W. Individual tasks related to the duty.Provides individual supportive counseling/therapy and group therapy for patients and their families.Carries out interventions in accordance with plan of service and modifies plans as necessary to further therapeutic goals.Educates relatives, interested persons and community agencies to the social aspects of mental illness.Provides education to patients, relatives, and guardians regarding aftercare needs and resources to maintain health and prevent re-hospitalization.Provides psychosocial rehabilitation groups (PSR) as part of the PSR program on each unit and assigned in the Individual Plan of Services.Duty 3General Summary of Duty 3% of Time 15 Functions as the Discharge Planning Coordinator.Individual tasks related to the duty.Maintains cooperative working relationships with CMHSP liaisons and agencies to facilitate placement of patients and independent living arrangements. Functions as a liaison to CMHSP staff to ensure continuity of care between the hospital and community agencies and awareness of treatment and aftercare plans. Coordinates the completion of Comprehensive Pre-Release plans and discusses patient’s needs with community representatives. Assists patients in the discharge and placement process by attempting to reduce resistance by patient and/or family through supportive counseling.Coordinates pre-placement visits with the CMHSP liaison when clinically appropriate. Overseas the discharge/release process until the departure of the patient. Develop discharge and aftercare plans with patient and/or family that clearly address the patient/ guardian's needs in the community.Coordinates the completion of discharge plans and documentation to assure continuity of services in the community.Duty 4General Summary of Duty 4% of Time15Documentation.Individual tasks related to the duty.Annotates services, activities and interventions in electronic medical pletes Progress Notes that are related to the goals in the IPOS and which reflect accurate progress of patient’s response to interventions in the IPOS provided by the social pletes Annual Psychosocial pletes all documentation required by community mental health necessary for discharge/placement continuity of pletes required documentation for all NGRI patients according to Standard Operating Procedures and Social Work Department procedures.Duty 5General Summary of Duty 5% of Time10Other assigned duties.Individual tasks related to the pletes petitions for probate court hearings and provides court testimony that includes clinically relevant and factual pletes appropriate documentation for guardianship proceedings as identified in the Standard Operating Procedures and Social Work Departmental procedures; may include completing petitions and court testimony.Duty 6General Summary of Duty 6% of TimeIndividual tasks related to the duty.16.Describe the types of decisions made independently in this position and tell who or what is affected by those decisions. EXPERIENCE:Clinical Social Worker 9 – No experience.Clinical Social Worker 10 – One year of professional experience as a clinical social worker.Clinical Social Worker 11 – Two years of professional experience as a clinical social worker. The desired candidate in this CSW-P-11 classification has a Master’s Degree in Social Work and a full Michigan Master’s Social Work License (LMSW).KNOWLEDGE, SKILLS, AND ABILITIES:?
