Ampicillin (Omnipen, Principen, Totacillin)

INDICATIONS: For the treatment of: Infections of the Genitourinary Tract (Including Gonorrhea), Infections of the Respiratory Tract, Infections of the Gastrointestinal Tract, and Meningitis.

DOSAGE AND ADMINISTRATION for Adults: Usual dose is 500 mg 4 times a day in equally spaced doses. For the treatment of gonerrhea in both men and women, a single oral dose of 3.5 grams of ampicillin administered simultaneously with 1 gram of probenecid is recommended.

Ampicillin should be taken with a full glass (8 oz) of water, one-half hour before or two hours after meals.

SIDE EFFECTS: Nausea, vomiting, diarrhea, skin rash, pruritus, urticaria.

CONTRAINDICATIONS: A history of a previous hypersensitivity reaction to any of the penicillins.

Aristocort (Triamcinolone, Aristopak)

INDICATIONS: Reduces swelling and decreases the body's immune response. Used to treat endocrine (hormonal) disorders when the body does not produce enough of its own steroids and to treat many immune and allergic disorders, such as arthritis and lupus.

DOSAGE AND ADMINISTRATION: Each dose should be taken with a full glass of water, or with food or milk to lessen stomach upset.

SIDE EFFECTS: Insomnia; nausea, vomiting, or stomach upset; fatigue or dizziness; muscle weakness or joint pain; problems with diabetes control; or increased hunger or thirst.

CONTRAINDICATIONS: Triamcinolone should not be taken with serious bacterial, viral, or fungal infections. Triamcinolone weakens the body's immune response and thus its ability to fight infection.

Benadryl (Diphenhydramine Hydrochloride Injection, USP)

INDICATIONS: For allergic reactions to blood or plasma, in anaphylaxis as an adjunct to epinephrine and other standard measures, for other uncomplicated allergic conditions of the immediate type when oral therapy is impossible or contraindicated, and for active treatment of motion sickness.

DOSAGE AND ADMINISTRATION: 10 to 50 mg intravenously at a rate generally not exceeding 25 mg/min, or deep intramuscularly, 100 mg if required; maximum daily dosage is 400 mg.

SIDE EFFECTS: Urticaria, drug rash, anaphylactic shock, photosensitivity, excessive perspiration, chills, dryness of mouth, nose, and throat.

CONTRAINDICATIONS: Should not be used in neonates, premature infants, or nursing mothers.

Celestone (Betamethazone)

INDICATIONS: For Endocrine, Rheumatic, and Hematologic disorders; Collagen, Dermatologic, Ophthalmic, Respiratory, Neoplastic, and Gastrointestinal diseases; and for Allergic and Edematous states.

DOSAGE AND ADMINISTRATION:The initial dosage of Celestone Syrup may vary from 0.6 mg to 7.2 mg per day depending on the specific disease entity being treated.

SIDE EFFECTS: May include sodium retention, fluid retention, muscle weakness, steroid myopathy, peptic ulcer, impaired wound healing; convulsions, menstrual irregularities.

CONTRAINDICATIONS: In systemic fungal infections.

This Medication should be Stored between 2° and 30°C (36° and 86°F).

Compazine (Prochlorperazine)

INDICATIONS: To control severe nausea and vomiting, for symptoms of schitzophrenia, and sometimes used for anxiety.

DOSAGE AND ADMINISTRATION: As a suppository or 5 - 10mg tablets. 3 - 4 times per day.

SIDE EFFECTS: Muscle spasms and twitches in the face and body, muscle rigdity.

CONTRAINDICATIONS: Antiseizure drugs, Anticoagulants, lithium, and narcotic painkillers.

This Medication should be Stored at room temperature away from heat and light.

DepoMedrol (Methylprednisolone)

INDICATIONS: For Endocrine Disorders, Rheumatic Disorders, Collagen Diseases, Dermatologic Diseases, Allergic States, Ophthalmic Diseases, Gastrointestinal Diseases, Respiratory Diseases, and others.

DOSAGE AND ADMINISTRATION: Intramuscular administration. For large joints, 20 to 80 mg; for medium joints, 10 to 40 mg; for small joints, 4 to 10 mg.

SIDE EFFECTS: May include sodium retention, fluid retention, muscle weakness, steroid myopathy, peptic ulcer, impaired wound healing; convulsions, menstrual irregularities.

CONTRAINDICATIONS: For intrathecal administration.

DepoProvera (Medroxyprogesterone)

INDICATIONS: Only for the prevention of pregnancy.

DOSAGE AND ADMINISTRATION: 150 mg of DEPO-PROVERA Contraceptive Injection every 3 months (13 weeks) administered by deep, IM injection in the gluteal or deltoid muscle.

SIDE EFFECTS: Menstrual irregularities, weight changes, headache, nervousness, abdominal pain or discomfort, dizziness, weakness or fatigue.

CONTRAINDICATIONS: Known or suspected pregnancy, undiagnosed vaginal bleeding, known or suspected malignancy of breast, active thrombophlebitis, history of thromboembolic disorders or cerebral vascular disease, liver dysfunction or disease, known hypersensitivity to DEPO-PROVERA Contraceptive Injection.

DepoTestosterone (Androderm, AndroGel, Testoderm)

INDICATIONS: to treat men for conditions that result from a lack of natural testosterone.

DOSAGE AND ADMINISTRATION: Patches are used on scrotal skin, which is very thin and absorbs the drug more easily. The patch is applied to clean, dry scrotal skin. The scrotal hair should be dry-shaved before a patch is applied. Each patch should be worn for 22 to 24 hours. Testosterone topical gel is applied once daily (preferably in the morning) to clean, dry, unbroken skin on the shoulders and upper arms or the abdomen as directed.

SIDE EFFECTS: itching, discomfort, redness, or irritation at the application site; headache; weakness; emotional changes; increased blood pressure; decreased interest in sex; nervousness; prostate changes; enlarged, swollen or tender breasts; or acne.

CONTRAINDICATIONS: Warfarin (Coumadin); oral diabetes medication such as glipizide (Glucotrol), glyburide (Diabeta, Micronase, Glynase), repaglinide (Prandin), troglitazone (Rezulin), rosiglitazone (Avandia), pioglitazone (Actos); propranolol (Inderal, Inderal LA, others); or corticosteroids such as betamethasone.

Store testosterone topical at room temperature away from moisture and heat.

Epinephrine (Adrenalin)

INDICATIONS: Treatment of acute hypersensitivity, treatment of acute asthmatic attacks, and treatment and prophylaxis of cardiac arrest.

DOSAGE AND ADMINISTRATION: 0.05 - 1mg intravenous injection.

SIDE EFFECTS: Anxiety, headache, fear and palpitations, cerebral hemorrhage, hemiplegia, subarachnoid hemorrhage, restlessness, throbbing headache, tremor, weakness dizziness, pallor and respiratory difficulty.

CONTRAINDICATIONS: Hypersensitivity to sympathomimetic amines, patients with angle closure glaucoma, patients in shock (nonanaphylactic), and patients anesthetized with agents such as cyclopropane or halothane.

Epoetin (Erythropoietin)

INDICATIONS: Treatment of Anemia of Chronic Renal Failure Patients

DOSAGE AND ADMINISTRATION: Starting dose is 50 - 150 units/kg. by intravenous injection.

SIDE EFFECTS: Hypertension, Headache, Arthralgias, Nausea, Edema, Fatigue, Diarrhea, Vomiting, Chest Pain.

CONTRAINDICATIONS: Uncontrolled hypertension, known hypersensitivity to mammalian cell-derived products, known hypersensitivity to Albumin (Human).

Glucagon

INDICATIONS: For treatment of severe hypoglycemia, and as a diagnostic aid.

DOSAGE AND ADMINISTRATION: 0.25 - 1.0mg intravenously.

SIDE EFFECTS: Nausea and vomiting.

CONTRAINDICATIONS: Patients with known hypersensitivity to it or in patients with known pheochromocytoma.

Heparin (Heparin Sodium Injection)

INDICATIONS: For anticoagulant therapy, prevention of post-operative deep venous thrombosis, prophylaxis and treatment of pulmonary embolism, atrial fibrillation with embolization, prevention of clotting in arterial and cardiac surgery, and as an anticoagulant in blood transfusions.

DOSAGE AND ADMINISTRATION: 5,000 to 10,000 Units should be given by intermittent intravenous injection every 4 - 6 hours.

SIDE EFFECTS: Hemorrhage and prolonged clotting time.

CONTRAINDICATIONS: Patients with severe thrombocytopenia, and those with an uncontrollable active bleeding state.

Hydralazine (Hydralazine hydrochloride)

INDICATIONS: For Essential hypertension.

DOSAGE AND ADMINISTRATION: 10 mg tablets four times daily for the first 2 to 4 days, then 25 mg four times daily for the first week. For the second and subsequent weeks, increase dosage to 50 mg four times daily.

SIDE EFFECTS: Headache, anorexia, nausea, vomiting, diarrhea, palpitations, tachycardia, angina pectoris.

CONTRAINDICATIONS: Hypersensitivity to hydralazine; coronary artery disease; mitral valvular rheumatic heart disease.

Imitrex (Sumatriptan Succinate)

INDICATIONS: For the acute treatment of migraine attacks in adults.

DOSAGE AND ADMINISTRATION: Single doses of 25, 50, or 100 mg

tablets. The dose may be repeated after 2 hours, not to exceed a total daily dose of 200 mg

SIDE EFFECTS: Warm or cold sensation; Chest pain, tightness, pressure, and/or heaviness; pain, tightness, or pressure in the neck, throat or jaw.

CONTRAINDICATIONS: Patients with history, symptoms, or signs of ischemic cardiac, cerebrovascular, or peripheral vascular syndromes. Also, patients with other significant underlying cardiovascular diseases.

Immune Globulin (HB) (BayGam)

INDICATIONS: To prevent against infestions related to Hepatitis A, Measles (Rubeola), Varicella, Rubella, and Immunoglobulin Deficiency.

DOSAGE AND ADMINISTRATION: 0.02 - 1.2 ml administered intramuscularly.

SIDE EFFECTS: Local pain and tenderness at the injection site, urticaria, and angioedema.

CONTRAINDICATIONS: Isolated immunoglobulin A (IgA) deficiency, and patients who have severe thrombocytopenia or any coagulation disorder that would contraindicate intramuscular injections.

Narcan (Naloxone)

INDICATIONS: For complete or partial reversal of narcotic depression, including respiratory depression; or for the diagnosis of suspected acute opioid overdosage.

DOSAGE AND ADMINISTRATION: 0.4 mg to 2 mg may be administered intravenously, intramuscularly, or subcutaneously.

SIDE EFFECTS: Nausea, vomiting, sweating, tachycardia, increased blood pressure, tremulousness, seizures and cardiac arrest.

CONTRAINDICATIONS: Contraindicated in patients known to be hypersensitive to it.

Penicillin (Bicillin) (Penicillin G Benzathine)

INDICATIONS: Mild-to-moderate infections of the upper-respiratory tract due to susceptible streptococci; venereal infections such as syphilis; rheumatic fever and/or chorea.

DOSAGE AND ADMINISTRATION: A single intramuscular injection of 1,200,000 units

SIDE EFFECTS: Hypersensitivity reactions, urticaria, laryngeal edema, fever, eosinophilia; and other serum sickness-like reactions.

CONTRAINDICATIONS: History of a previous hypersensitivity reaction to any of the penicillins.

Progesterone (Prometrium)

INDICATIONS: For use in the prevention of endometrial hyperplasia in non-hysterectomized postmenopausal women who are receiving conjugated estrogens tablets. Also for use secondary amenorrhea.

DOSAGE AND ADMINISTRATION: Capsules are given as a single daily dose in the evening, 200 mg orally for 12 days sequentially per 28 day cycle, to postmenopausal women with a uterus who are receiving daily conjugated estrogens tablets for the prevention of endometrial hyperplasia. Capsules may be given as a single daily dose of 400 mg in the evening for 10 days for secondary amenorrhea.

SIDE EFFECTS: Headache, Breast Tenderness, Joint Pain, Depression, Dizziness, Abdominal Bloating, Hot Flashes, and Urinary Problems.

CONTRAINDICATIONS: Known sensitivity, known or suspected pregnancy, thrombophlebitis, thromboembolic disorders, cerebral apoplexy, severe liver dysfunction or disease, known or suspected malignancy of breast or genital organs, undiagnosed vaginal bleeding.

Rochepin ()

Vistaril (Hydroxyzine pamoate)

INDICATIONS: For symptomatic relief of anxiety and tension associated with psychoneurosis, as an adjunct in organic disease states in which anxiety is manifested, and useful in the management of pruritus due to allergic conditions such as chronic urticaria and atopic and contact dermatoses, and in histamine-mediated pruritus.

DOSAGE AND ADMINISTRATION: 25 - 100 mg capsules 3 - 4 times a day. When treatment is initiated by the intramuscular route of administration, subsequent doses may be administered orally.

SIDE EFFECTS: Transient drowsiness and involuntary motor activity.

CONTRAINDICATIONS: Patients who have shown a previous hypersensitivity to it.

Vitamin B (Thiamine)

INDICATIONS: To prevent and to treat deficiencies of thiamine.

DOSAGE AND ADMINISTRATION: Take each dose with a full glass of water.

SIDE EFFECTS: A feeling of warmth, itching or hives, weakness, sweating, nausea, and restlessness.

CONTRAINDICATIONS: None known.

Store thiamine at room temperature away from moisture and heat.

Vitamin B 12 (Cyanocobalamin Injection)

INDICATIONS: For vitamin B12 deficiencies due to malabsorption which may be associated with Addisonian (pernicious) anemia; Gastrointestinal pathology, dysfunction, or surgery; fish tapeworm infestation; malignancy of pancreas or bowel; or folic acid deficiency.

DOSAGE AND ADMINISTRATION: A dose of 100 mcg daily for 6 or 7 days should be administered by intramuscular or deep subcutaneous injection.

SIDE EFFECTS: Polycythemia vera, mild transient diarrhea, itching; transitory exanthema, or a feeling of swelling of the entire body

CONTRAINDICATIONS: Sensitivity to cobalt or vitamin B12.

Vitamin K (Phytonadione)

INDICATIONS: Coagulation disorders such as, anticoagulant-induced prothrombin deficiency; prophylaxis and therapy of hemorrhagic disease of the newborn; hypoprothrombinemia due to antibacterial therapy.

DOSAGE AND ADMINISTRATION: 2.5 - 10 mg or up to 25 mg by the subcutaneous or intramuscular route. To be injected very slowly, not exceeding 1 mg per minute.

SIDE EFFECTS: Transient "flushing sensations" and "peculiar" sensations of taste have been observed, as well as rare instances of dizziness, rapid and weak pulse, profuse sweating, brief hypotension, dyspnea, and cyanosis.

CONTRAINDICATIONS: Hypersensitivity to any component of this medication.

Protect from light at all times.

K-Dur (Potassium Chloride)

INDICATIONS: For the treatment of patients with hypokalemia and for the prevention of hypokalemia.

DOSAGE AND ADMINISTRATION: 50 to 100 mEq per day in extended-release capsules or tablets

SIDE EFFECTS: Nausea, vomiting, flatulence, abdominal pain/discomfort, and diarrhea.

CONTRAINDICATIONS: In patients with hyperkalemia and it may produce esophageal ulceration in certain cardiac patients with esophageal compression due to an enlarged left atrium.

The solid oral dose is contraindicated in any patient in whom there is structural, pathological or pharmacologic cause for arrest or delay in tablet or capsule-passage through the gastrointestinal tract.

Demerol (Meperidine hydrochloride)

INDICATIONS: For the relief of moderate to severe pain, for preoperative medication, for support of anesthesia, and for obstetrical analgesia.

DOSAGE AND ADMINISTRATION: 50 mg to 150 mg intramuscularly, subcutaneously, or orally, every 3 or 4 hours as necessary.

SIDE EFFECTS: Lightheadedness, dizziness, sedation, nausea, vomiting, and sweating.

CONTRAINDICATIONS: Contraindicated in patients who are receiving monoamine oxidase (MAO) inhibitors.

Toradol (Ketorolac tromethamine)

INDICATIONS: For the short-term (≤5 days) management of moderately severe, acute pain that requires analgesia at the opioid level (usually in a postoperative setting).

DOSAGE AND ADMINISTRATION: 15-30 mg as a single or multiple dose, on a regular or "pm" schedule. Intravenous or intramuscularly. The maximum daily dose should not exceed 60 mg. Combined duration of use is not to exceed 5 days.

SIDE EFFECTS: Edema, hypertension, pruritus, rash, nausea, dyspepsia, gastrointestinal pain; diarrhea, constipation, flatulence, gastrointestinal fullness, vomiting, stomatitis, headache, drowsiness, dizziness, sweating. and injection-site pain.

CONTRAINDICATIONS: Peptic ulcer disease, recent gastrointestinal bleeding or perforation, patients with a history of peptic ulcer disease or gastrointestinal bleeding. Patients with advanced renal impairment, or at risk of renal failure due to volume depletion. In labor and delivery because it may adversely affect fetal circulation and inhibit uterine contraindications. In nursing mothers because of the potential adverse effects. In patients with previously demonstrated hypersensitivity to ketorolac tromethamine, or allergic manifestations to aspirin or other non-steroidal anti-inflammatory drugs.

Valium (Diazepam)

INDICATIONS: For the management of anxiety disorders or for the short-term relief of the symptoms of anxiety.

DOSAGE AND ADMINISTRATION: 2 mg to 20 mg IM or IV, depending on the indication and its severity.

SIDE EFFECTS: Drowsiness, fatigue and ataxia; venous thrombosis and phlebitis at the site of injection.

CONTRAINDICAITONS: Known hypersensitivity to this drug; acute narrow angle glaucoma; open angle glaucoma, and known hypersensitivity to soy protein.

Ketorolac (Toradol)

INDICATIONS: For the short-term (≤5 days) management of moderately severe, acute pain that requires analgesia at the opioid level (usually in a postoperative setting).

DOSAGE AND ADMINISTRATION: The recommended dose is 30 mg ketorolac tromethamine by IV or IM every 6 hours. The maximum daily dose should not exceed 120 mg.

SIDE EFFECTS: Edema, hypertension, pruritus, rash, nausea, dyspepsia, gastrointestinal pain, diarrhea, constipation, flatulence, gastrointestinal fullness, vomiting, stomatitis, headache, drowsiness, dizziness, sweating and pain at the injection site.

CONTRAINDICATIONS: Patients with active peptic ulcer disease, in patients with recent gastrointestinal bleeding or perforation, and in patients with a history of peptic ulcer disease or gastrointestinal bleeding. Patients with advanced renal impairment, or in patients at risk of renal failure due to volume depletion. In labor and delivery. In nursing mothers. Patients with previously demonstrated hypersensitivity to ketorolac tromethamine, or allergic manifestations to aspirin or other non-steroidal anti-inflammatory drugs. As a prophylactic analgesic before any major surgery is critical because of the increased risk of bleeding. In patients with suspected or confirmed cerebrovascular bleeding, hemorrhagic diathesis, incomplete hemostasis and those at high risk of bleeding. Patients currently receiving ASA or N.A.D.

Tylenol with Codeine (Tylenol/Cod)

INDICATIONS: For the relief of mild to moderately severe pain.

DOSAGE AND ADMINISTRATION: A single dose of codeine phosphate is 15 - 60 mg, and not to exceed 360 mg per day. A single dose of acetaminophen is 300 - 1000 mg, and not to exceed 4000 mg per day.

SIDE EFFECTS: The most frequent side effects include lightheadedness, dizziness, sedation, shortness of breath, nausea, and vomiting

CONTRAINDICATIONS: Acetaminophen and codeine phosphate tablets, oral solution , or capsules, should not be taken by patients who have shown previous hypersensitivity to any component.

Percocet (Oxycodone and Acetaminophen)

INDICATIONS: for the relief of moderate to moderately severe pain

DOSAGE AND ADMINISTRATION: Dosage should be adjusted according to the severity of the pain and the response of the patient. The usual adult dosage is 1 capsule or 1 tablet every 6 hours as needed for pain.

SIDE EFFECTS: Lightheadedness, dizziness, sedation, nausea, and vomiting.

CONTRAINDICATIONS: Patients who are hypersensitive to oxycodone or acetaminophen.

Capsules and tablets should be kept at a controlled room temperature of 59-86°F, and should be protected from moisture.

Vicodin (Hydrocodone and Acetaminophen)

INDICATIONS: For the relief of moderate to moderately severe pain.

DOSAGE AND ADMINISTRATION: For the 500 mg of acetominophen/5 mg hyrdocodone tablet, one or two tablets every four to six hours as needed for pain. The total 24 hour dose should not exceed 8 tablets. For the 650 mg/7.5 mg tablet, one tablet every four to six hours as needed for pain. The total 24 hour dose should not exceed 5 tablets.

SIDE EFFECTS: Lightheadedness, dizziness, sedation, nausea, and vomiting

CONTRAINDICAITONS: Hypersensitivity to acetaminophen or hydrocodone.

Tablets are to be stored at a controlled room temperature of 59° to 86° F.

Lidocaine (Lidocaine Viscous)

INDICATIONS: For the production of topical anesthesia of irritated or inflamed mucous membranes of the mouth and pharynx. It is also useful for reducing gagging during the taking of X-ray pictures and dental impressions.

DOSAGE AND ADMINISTRATION: The maximum recommended single dose of Lidocaine should not exceed 2 mg/lb body weight and does not in any case exceed a total of 300 mg.

SIDE EFFECTS: Lightheadedness, nervousness, apprehension, euphoria, confusion, dizziness, drowsiness, tinnitus, blurred or double vision, and vomiting.

CONTRAINDICATIONS: Patients with a known history of hypersensitivity to local anesthetics of the amide type.

Xylocaine (Lidocaine)

INDICATIONS: For production of local or regional anesthesia by infiltration techniques such as percutaneous injection and intravenous regional anesthesia by peripheral nerve block techniques such as brachial plexus and intercostal and by central neural techniques, like lumbar and caudal epidural blocks

DOSAGE AND ADMINISTRATION: 5 - 300 mg given percutaneously or intravenously. Maximum dose should not exceed 3.5 mg/lb of body weight.

SIDE EFFECTS: Lightheadedness, nervousness, apprehension, euphoria, confusion, dizziness, drowsiness, tinnitus, blurred or double vision, and vomiting

CONTRAINDICAITONS: Patients with a known history of hypersensitivity to local anesthetics of the amide type.

Lidocaine with Epinephrine (LidoPen, Xylocaine)

INDICATIONS: Inhibits conduction of nerve impulses, causing temporary loss of feeling and sensation.

DOSAGE AND ADMINISTRATION: 10 - 300 mg IV or IM.

SIDE EFFECTS: Pain at injection site, burning, stinging and tenderness.

CONTRAINDICATIONS: Hypersensitivity to amide-type local anesthetics, Adams-Stokes syndrome, superventricular arrythmias, Wolff-Parkinson-White syndrome.

Albuterol (Ventolin)

INDICATIONS: For the prevention and relief of bronchospasm in patients with reversible obstructive airway disease and for the prevention of exercise-induced bronchospasm.

DOSAGE AND ADMINISTRATION: Two inhalations of the aerosol repeated every 4 to 6 hours.

SIDE EFFECTS: Tremor, nausea, tachycardia, palpitations, nervousness, increased blood pressure, dizziness, and heartburn.

CONTRAINDICATIONS: Patients with a history of hypersensitivity to albuterol or any of the formulation components.

Atrovent (Ipratropium Bromide)

INDICATIONS: either alone or with other bronchodilators, especially beta adrenergics as a bronchodilator for maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease, including chronic bronchitis and emphysema.

DOSAGE AND ADMINISTRATION: 500 mcg (1 Unit-Dose Vial) administered three to four times a day by oral nebulization, with doses 6 to 8 hours apart.

SIDE EFFECTS: Headache, pain, influenza-like symptoms, back pain, chest pain, and nausea.

CONTRAINDICATIONS: Patients with a history of hypersensitivity to soya lecithin or related food products such as soybean and peanut, and in known or suspected cases of hypersensitivity to ipratropium bromide, or to atropine and its derivatives.

DPT (Diptheria, Pertussis, and Tetanus)

USE: This vaccine protects against Diptheria, Pertussis, and Tetanus.

DOAGE AND ADMINISTRATION: 0.5 ml is given intramuscularly. For children it is given at 2, 4, 6, and 15-18 months of age; then a fifth injection is given at 4-6 years-old. DT (Diptheria and Tetanus) is given at 11-12 years of age and every 10 years after.

DOSAGE AND ADMINISTRATION: Given subcutaneously in infants at 2, 4, and 6-18 months of age, with a booster given at 4-6 years. Adults can recieve the first dose at any time, recieve the second 1-2 months after the first, and then the third at 6-12 months after the second.

SIDE EFFECTS: Soreness at site of injection.

CONTRAINDICATIONS: Anyone who has had life-threatening allergic reaction to the antibiotics neomycin, streptomycin, or polymyxin B. Anyone who's had a severe allergic reaction to a previous polio shot.

Hib (Haemophilus Influenzae Type B Vaccine)

USE: This vaccine protects against Haemophilus Influenzae Type B disease.

DOSAGE AND ADMINISTRATION: Given intramuscularly to infants only at 2, 4, 6, and 12-15 months of age.

SIDE EFFECTS: Redness, warmth, or swelling at injection site; fever over 101 degrees F.

CONTRAINDICATIONS: Patients who've had a life-threatening allergic reaction to a previous dose, infants less than 6 weeks of age, patients who are moderately or severely ill at the scheduled time of the shot.

MMR (Measles, Mumps, Rubella)

USE: This vaccine protects against the serious diseases of Measles, Mumps, and Rubella.

DOSAGE AND ADMINISTRATION: Two doses are given subcutaneously in children, the first at 12-15 months of age, the second at 4-6 years. Adults recieve just one dose.

SIDE EFFECTS: Fever, mild rash, swelling of cheeck or neck glands.

CONTRAINDICATIONS: Pregnant women, and patients who've experienced life-threatening allergic reactions to gelatin, the neomycin antibiotic, or a previous dose of the MMR vaccine.

Varicella (Chickenpox Vaccine, Varivax)

USE: This vaccine can prevent chickenpox, a common childhood disease that can be especially serious in young infants and adults.

DOSAGE AND ADMINISTRATION: One dose is given intramuscularly to infants at 12-18 months of age. Or two doses are given to adults 4-8 weeks apart.

SIDE EFFECTS: Soreness or swelling at injection site, fever, mild rash for up to one month after vaccination.

CONTRAINDICATIONS: Previous life-threatening reactions to gelatin, the neomycin antibiotic, or a previous dose of the Varicella vaccine.

Hepatitis A (HAV)

USE: This vaccine protects against a serious liver disease cause by the hepatitis A virus and is given to persons who live in high risk locations..

DOSAGE AND ADMINISTRATION: Two doses are give intramuscularly at least 6 months apart.

Hepatitis B (HBV)

DOSAGE AND ADMINISTRATION: Three doses are given intramuscularly. In infants they are given at birth, 1 month of age, and 6 months. In children of 6 the first dose is given anytime, the second 1 month later and the third dose 6 months after the first.

SIDE EFFECTS: Soreness at injection site, mild to moderate fever.

CONTRAINDICATIONS: Life-threatening allergic reactions to baker's yeast or a previous dose of the HBV vaccine.

Pneumocococcal Polysaccharide (Prevnar, PPV)

USE: This vaccine protects against a 23 types of pneumococcal bacteria than can lead to a serious disease called Pneumococcal disease.

DOSAGE AND ADMINISTRATION: Given subcutaneously at age 65 or to anyone at high risk over the age of 2. Any initial dose given before the age of 65 requires a booster every 5 years. If given under the age of 9, a second dose is given 4 weeks after the first dose.

SIDE EFFECTS: Redness and pain at injection site, fever, and muscle aches.

CONTRAINDICATIONS: None known.

Meningococcal

USE: Given to persons at risk of getting meningococcal disease, a serious illness cause by a bacteria. Not routinely given.

DOSAGE AND ADMINISTRATION: 2 doses 3 months apart for infants aged 3 months to 2 years. 1 dose for people 2 years of age and over.

SIDE EFFECTS: REdness or pain at injection site may last for 1 -2 days.

CONTRAINDICATIONS: Anyone who has had a serious allergic reaction to a previous dose of the vaccine, people who are mildly ill at the time the vaccine is scheduled to be administered.

Oral Polio Vaccine (OPV)

USE: This vaccine gives immunity to polio. The oral form is better at keeping the disease from spreading to othre people.

DOSAGE AND ADMINISTRATION: Drops of a live form of the polio vaccine that are swallowed.
