2022-2023 K-8 Tuition Scholarships Application

? This application is required for any family wishing to receive a K-8 tuition scholarship. ? Please complete the section of this application for the scholarship you wish to receive. ? Only one scholarship may be applied to a student's tuition. ? Current families please include completed application with Re-Enrollment.

HOLY FAMILY SCHOLARSHIP

To be eligible for this scholarship a parent and their child/ren must be baptized and practicing Catholics, registered parishioners of a Catholic Church in the Diocese of St. Augustine, and in good standing with Holy Family Catholic School from previous academic year. Students enrolled in the Preschool programs do not qualify under this scholarship.

A practicing Catholic is defined as one who follows the precepts of the Church as stated in the Catechism of the Catholic Church (2041-2043). These precepts are the "very necessary minimum" and are outline below.

1. Attend Mass on Sundays and on holy days of obligation. 2. Confess your sins in the sacrament of Reconciliation at least once a year. 3. Receive the sacrament of the Eucharist at least during the Easter season. 4. Observe the days of fasting and abstinence established by the Church.

a. Fasting on Ash Wednesday and Good Friday. b. Abstain from eating meat on Ash Wednesday and Fridays of Lent. 5. Help to provide for the needs of the Church, each according to your own ability.

ST. AUGUSTINE SCHOLARSHIP

To be eligible for this scholarship a parent and their child/ren must be baptized and registered members of St. George Antiochian Orthodox Church, and in good standing with Holy Family Catholic School from previous academic year. Students enrolled in the Preschool programs do not qualify under this scholarship.

Florida Tax Credit (FTC), Family Empowerment-Education Opportunity (FES-EO), Family Empowerment Scholarship-Unique Abilities (FES-UA (Formerly known as Gardiner or McKay).

Families who are awarded any of these scholarships must provide a copy of their award letter from the scholarship granting organization to the school office before it will be reflected in the FACTS Tuition Management System.
