1. Who is included in this directory? A. This directory includes all physicians and mid-level providers who are active participants in Hackensack Meridian Health Partners, Hackensack Meridian Health's clinically integrated network.

2. How to access this directory online? A. Visit to access the HMHP provider directory. You can also access the directory by visiting: o and selecting Find a Doctor o and clicking on Find an HMHP Provider

3. How often is this directory updated? A. New HMHP providers are loaded into the directory within 24 hours. Updates to existing provider profiles are completed every few days. Updates that require verification will take longer to appear in the online directory.

4. I don't have a provider photo. How do I have my photo taken (or updated)? A. Contact your local Hackensack Meridian Health hospital's medical staff office for additional photo information.

5. Will this directory include additional information in the future? A. The Hackensack Meridian Health Partners Directory will continue to be refined based upon provider and patient need. If you have feedback that you would like to share, please use the Update Request link on your provider profile and select `Other.'

6. How do I print out provider information? A. To print out provider information, click the `Download All Results' button underneath the `Search by Location' bar. Once clicked, you can either download your results in a CSV (Excel) spreadsheet format or a PDF format, which includes the physician's photo. B.

Please note that it takes three seconds to load every 50 providers so if you print multiple provider results the download process could take a minute or two to complete.

7. My information is incorrect. How can I have it updated? A. Please scroll to the bottom of the outdated profile and click on the Provider Update Form button, located on the right hand side, and fill out the update form. To update a provider's board certifications, specialty and education (ie: fellowship, residency), please contact your medical staff office.

8. I'm in HMHP, but I'm not listed in the directory. How can I be added? A. Please click on and complete the Provider Update Form located on the left side of the directory's main page under the Additional Filters section.
