Your enrollment period to enroll in benefit elections is 30 days from your benefit effective date (1st of the month following your date of hire or date you became benefits eligible)

To start the enrollment process, please go online to access the site at hallmarkhealth..

Enter your Username and Password

Username: Enter the first initial of your first name, your full last name and the last four digits of your Social Security number (example: John Doe = jdoe6575).

Password: Enter the last four digits of your Social Security number.

Enrollment is processed in four easy steps:

Step 1: Review and verify your personal information.

Step 2:

Review and verify your family (dependents) information. All dependents must be verified and the required documentation must be submitted before coverage will be activated. (See acceptable documentation per dependent sheet).

Step 3: Answer the question regarding your tobacco usage.

Step 4:

Enroll in your benefit selections. HINT: Utilize the "Best Fit Plan Advisor" or "Compare Costs" tools to ensure you have selected the plan that works best for you and your family!

Step 5:

Confirm your benefit selections for the 2018 plan year by clicking the "I agree to the changes I have made and I am finished with my enrollment", then click on the "Save My Enrollment!" button!

It's that easy! It is highly recommended that you e-mail yourself (or print) a confirmation of your elections. To do so, click on the Envelope Icon on the New Elections page after making your elections. In addition, a confirmation of your elections will be mailed to your home address.
