2019 SENIOR AWARDS

The following is the list of locally sponsored awards.

** All awards and amounts are tentative and may be subject to change. **

Recipients of the awards and their parents will be invited to attend the Senior Recognition Night on Tuesday, May 21, 2019 at 6:00 p.m. in the high school gymnasium.

DIRECTIONS for Awards #1-#31:

Review the eligibility requirements for each of the awards listed below and decide which pertain to you. Print out the Senior Award Application Form. Complete just one application form and return it to Mrs. Bedford in the Guidance Office, letting her know how many awards you are applying for. She will make enough copies for you, and then you will write the name and number of each award on the application copies. Make sure to complete the application form in its entirety. If you have questions about the form, see your School Counselor. Award recipients will be announced on Senior Recognition Night.

Awards #32-#46 have their own unique applications and/or supplemental requirements, each listed separately.

Application Help Sessions will be made available during X-Periods in February. Please watch to UPN for dates.

DEADLINE: Completed applications for all awards are due Friday, March 1st, 2019. Return them to the Guidance Office.

1. AMERICAN LEGION AMERICANISM AWARD

A. Scholarship amount: $250.00

B. Eligibility: 1. Student who displays honor and service to the school and community and who plans to enter the United States military service.

2. AMERICAN LEGION AUXILIARY PERKIOMEN UNIT 184 AWARD

A. Scholarship amount: $500.00

B. Eligibility: 1. A senior girl continuing her education in the field of nursing.

3. GORDON H. BAVER, INC. SCHOLARSHIP

A. Scholarship amount: $250 for each of 2 years

B. Eligibility: 1. Awarded to a student who is planning to further his/her education in advanced

study leading to an associate degree as a craftsman as it relates to the building

construction industry.

2. Based on potential for success and good character.

4. LILA K. BITTENBENDER NURSING SCHOLARSHIP

A. Scholarship amount: $1000.00

B. Eligibility: 1. Recipient must be preparing for a career in nursing.

2. High academic achievement.

3. Service to school and community.

5. THE M. GEORGIANN BUTLER AWARD

A. Scholarship amount: $1000.00

B. Eligibility: 1. Economic need.

2. Involvement in any extracurricular activities or community organizations/activities.

3. Planning to attend ANY post-secondary education (including trade school, community college or four-year).

6. CHORUS ORCHESTRA BOOSTERS – STRING ORCHESTRA AWARD

A. Scholarship Amount: $250.00

B. Eligibility: 1. To be awarded to a student who is planning to further his/her education related to string

Instrumental music.

7. CHORUS ORCHESTRA BOOSTERS – VOCAL MUSIC AWARD

A. Scholarship amount: $250.00

B. Eligibility: 1. To be awarded to a student who is planning to further his or her education related to vocal music.

8. COMMONWEALTH INSURANCE GROUP T/A DURN & BIELER INSURANCE AGENCY SCHOLARSHIP

A. Scholarship amount: $1000.00

B. Eligibility: 1. Strong need for financial assistance.

2. Going into the field of special education or working in the special needs community.

3. Strong academic achievement.

4. Active in community activities.

9. EAST GREENVILLE FIRE CO. AUXILIARY VOLUNTEERISM AWARD

A. Scholarship amount: $250

B. Eligibility: 1. Awarded to a senior who is actively involved in serving our community ie. fire department, emergency medical service, community support services.

2. Demonstrates strong character and dedication to volunteer work.

3. Including all seniors regardless of the pursuit of a higher education.

10. BARRY J. FETTERMAN MEMORIAL AWARD

A. Scholarship Amount: $100

B. Eligibility: 1. Any student continuing education after graduation.

2. Minimum GPA of 3.0.

3. 3. Must have participated in varsity athletics.

4. Personal characteristics of leadership, dedication and loyalty.

11. FOR THE LOVE OF LORI TRUST MEMORIAL SCHOLARSHIP COMMENCEMENT AWARD A. Scholarship amount: $1,000.00

B. Eligibility: 1. Awarded to a senior girl who has been accepted in a four year college program, preference to a major in science at a liberal arts college.

2. Achieved academic and athletic excellence.

3. Active in school activities, preference to band and chorus.

4. Demonstrates strong character, integrity and leadership.

5. Regarded as a pleasant, friendly, cooperative and genuine student by peers, faculty and staff.

12. LT. WILLIAM GENDEBIEN MEMORIAL AWARD

A. Scholarship amount: $500.00

B. Eligibility: 1. Senior who exhibits patriotism and dedication to the American ideal.

2. Continuing education in social sciences.

3. Service to school and/or community.

13. GREEN LANE/MARLBOROUGH LIONESS CLUB AWARD

A. Scholarship amount: $500.00

B. Eligibility: 1. To be awarded to a senior girl who has been an active participant in the Community Service course at UPHS.

2. Plans to continue her education in any manner of post-high school education.

14. GULACK FOUNDATION SCHOLARSHIP

A. Scholarship Amount: Approximately $8,250.00 scholarship, $2,062.50 payable per year for each of four years

B. Eligibility: 1. Accepted in a 4 year college program.

2. Selection based on financial need.

15. SP4 RAY IRA HAAS MEMORIAL SCHOLARSHIP

A. Scholarship amount: $500.00

B. Eligibility: 1. A student who displays honor and service to the school, community and who plans to enter the United States Military Service.

2. A Senior who exhibits patriotism and dedication to the American ideal.

3. Demonstrated need of financial assistance.

16. NESTLER FOUNDATION SCHOLARSHIP

A. Scholarship amount: approx. $760.00

B. Eligibility: 1. The student needs to have been enrolled at Upper Perkiomen H.S.

at least for two years.

2. Funds will not be given for aid to a student to be enrolled in the ministry or religious work careers.

17. PERKIOMEN LODGE #595 YOUTH SCHOLARSHIP AWARD

A. Scholarship amount: $500.00

B. Eligibility: 1. A senior boy who is proficient in Music, Mathematics, or English.

2. Minimum GPA 3.25;

3. Demonstrates good citizenship and compassion for other students;

4. Intending to pursue a post-secondary education in the Liberal Arts,

Engineering, or a Vocational/Technical trade.

18. PERKIOMEN VALLEY WOMAN’S CLUB AWARD

A. Scholarship amount: 2 Awards of $2000.00 each

B. Eligibility: 1. Senior who is in upper 5th of class rank.

2. Active in extra-curricular activities.

3. Desirable traits: personality, amiability, sincerity, loyalty, and cooperation.

19. THE ANDREW J. and C. JOAN PISTORIA SCHOLARSHIP

A. Scholarship amount: $500.00

B. Eligibility: 1. Student will be attending a 4-year accredited institution;

2. Outstanding scholarship with a GPA of at least 3.5;

3. Participation in extracurricular activities;

4. Preference to a 1st generation college student.

20. ANNE K. RAYMOND FINE ARTS AWARD

A. Scholarship amount: $2000.00

B. Eligibility: 1. To be awarded to a student continuing their education in the area of the fine arts.

2. Must have been involved with the drama club at UPHS or some other theatrical production.

21. RED HILL BAND INSTRUMENTAL MUSIC AWARD

A. Scholarship amount: $750.00

B. Eligibility: 1. Awarded to a student who can demonstrate two years of membership in the Upper Perkiomen High School’s marching or concert band. One of these years must be their senior year.

2. Will be furthering his/her education in the field of music

3. Can demonstrate current participation in a community musical organization

22. WILSON R. RITTER MEMORIAL AWARD

A. Scholarship amount: $500.00

B. Eligibility: 1. Shall have attended UPHS for at least two years.

2. Have demonstrated good citizenship qualities while in high school.

3. Have a need for financial assistance.

4. Award may be used toward the expenses of any type of post-high school training.

23. ST. LUKE’S QUAKERTOWN CAMPUS BOARD OF TRUSTEES ACADEMIC SCHOLARSHIP

A. Scholarship amount: approx. $500.00

B. Eligibility: 1. Be in the top 25% of the graduating class.

2. Be enrolled as a full time student in an accredited four year college or university in the field of Health Care or Science.

24. ST. LUKE’S QUAKERTOWN CAMPUS MEDICAL STAFF ACADEMIC SCHOLARSHIP

A. Scholarship amount: approx. $500.00

B. Eligibility: 1. Be in the top 25% of the graduating class.

2. Be enrolled as a full time student in an accredited four year college or university in the field of Health Care or Science.

25. The “TIP” AND POLLY STEVENS SCHOLARSHIP

A. Scholarship amount: $200.00

B. Eligibility: 1. Awarded to someone who has exhibited sound academic achievement. 2. Have a need for financial assistance.

3. Demonstrates a willingness to help their fellow student.

26. TRI-COUNTY AREA FEDERAL CREDIT UNION SCHOLARSHIP

A. Scholarship amount: $500.00

B. Eligibility: 1. Academic achievement – 3.0 GPA minimum;

2. Community involvement and activities;

3. Leadership;

4. Future endeavors that include majoring in Business, Accounting, and/or Finance.

27. UPPER PERKIOMEN VALLEY LIONS CLUB SCHOLARSHIP

A. Scholarship amount: $1,000.00.

B. Eligibility: 1. Active participation in the Leo Program at UPHS for a minimum of 2 years.

2. Plans to continue his or her education in any manner of accredited post high school education.

3. Exhibited sound academic achievement and has financial need.

28. UPPER PERKIOMEN VALLEY LIONS CLUB EXTRACURRICULUAR SCHOLARSHIP

A. Scholarship amount: $500.00

B. 1. Financial need;

2. Plans to continue his or her education in any manner of accredited post-high school

Education;

3. Active involvement in extracurricular school activity (i.e. band, arts, athletics, etc.).

29. UPHS CLASS OF 1965 MEMORIAL SCHOLARSHIP

A. Scholarship amount: $500.00

B. Eligibility: 1. Senior accepted in an accredited trade/technical school (can include trade/technical major at a community college or business school);

2. Financial need;

3. Commitment to complete training;

4. Satisfactory academic achievement.

30. UPPER PERKIOMEN HIGH SCHOOL CLASS OF 1969 AWARD

A. Scholarship amount: $200.00

B. Eligibility: 1. 3.0 GPA or higher;

2. Involved in extracurricular activities such as athletics, band, chorus, community service, and volunteer work;

3. Recipients must be attending some type of post-secondary education.

31. UPPER PERKIOMEN HS CLASS OF 1980 YOUTH SCHOLARSHIP AWARD

A. Scholarship amount: $75.00

B. Eligibility: 1. Graduating senior student pursuing post-secondary academic or vocational education;

2. Minimum GPA of 3.00;

3. Active involvement in community organization and extra-curricular activities

4. Demonstrates need for financial assistance.

DIFFERENT APPLICATION FORMS ARE REQUIRED FOR THE FOLLOWING AWARDS. THEY CAN BE OBTAINED IN THE GUIDANCE OFFICE.

32. BROWN PRINTING LEGACY SCHOLARSHIP

A. Scholarship Amount: 2 Awards of $1000.00

B. Eligibility: 1. Pursuing a post-secondary academic or vocational education in the science, technology, engineering, and/or mathematics fields;

2. Residing in the Greater Upper Perkiomen Valley;

3. Maintaining above a “B” average;

4. Demonstrating leadership through participation in school and/or extracurricular activities;

5. Providing examples of active community service.

Brown Printing Legacy application is a separate form which may be obtained in the Guidance Office.

Deadline: Friday March 1st, 2019

33. BUSINESS AND PROFESSIONAL WOMEN’S CLUB SCHOLARSHIP AWARD

A. Scholarship amount: approx. $1500.00

B. Eligibility:

1. Senior girl who has been accepted in a four-year college program.

2. Minimum 3.0 GPA.

3. Needs financial assistance.

4. Demonstrating leadership through participation in school and/or extracurricular activities.

5. Essay – 300-word min./500-word max. on “How the student will apply education towards empowerment of women in the future”.

The Business and Professional Women’s Club Scholarship Award application

is a separate form which may be obtained in the Guidance Office.

Deadline: Friday March 1st, 2019

34. GARY LESTER KELLER MEMORIAL AWARD

A. Scholarship amount: $500.00

B. Eligibility: Must meet at least one of the following criteria:

1. A senior that has experienced some adversity in his/her life;

2. Is an active volunteer in the community;

3. Is an inspiration to his/her family and friends.

Gary Keller Memorial Award application

is a separate form which may be obtained in the Guidance Office.

35. KEITH KLEPAC MEMORIAL AWARD

A. Scholarship amount: approx. $750.00

B. Eligibility: 1. Awarded to a senior who aspires to attend any post-secondary education program. Any student can apply but preference is given to a learning support student.

2. Demonstrates a willingness to befriend their fellow student.

3. Is regarded as a charismatic, pleasant, and genuine individual by peers, faculty, and staff.

Keith Klepac Scholarship application is a separate form which may be obtained in the Guidance Office.

36. THE EDMUND K. KUHNS/UPHS BAND BOOSTER SCHOLARSHIP AWARD

A. Scholarship Amount: $500.00

B. Eligibility: 1. Completion of the Band Booster Scholarship Application which will include:

a. Documented acceptance into a 2 or 4 year accredited collegiate or trade school;

b. Proof of a cumulative average GPA of no less than 3.0 through the 3rd marking period of their senior year as confirmed by the High School Guidance office.

2. The student actively participated in Marching and/or Concert and/or Jazz Band for at least two years of high school, one of which must have been their senior year. The student has actively participated in fundraising activities supported by the Band Boosters.

3. Award recipients will be determined by their participation, dedication, work ethic, attitude and leadership within the UPHS band program.

The UPHS Band Booster Scholarship application is separate and can be obtained in the UPHS Guidance Office.

37. ROXANNE R. MAY MEMORIAL SCHOLARSHIP FUND

A. Scholarship Amount: $500.00

B. Eligibility: 1. A vocational technical school student with preference given to a cosmetology student.

2. The student will continue education/requirements towards certification after graduation.

The Roxanne R. May Memorial Scholarship application is a separate form and can be obtained in the UPHS

Guidance Office.

Deadline: Friday March 1st, 2019

38. BEN OBERHOLTZER MEMORIAL SCHOLARSHIP

A. Scholarship amount: approximately $1000.00

B. Eligibility: 1. Recipient plans to further education in visual arts, design, graphic arts or creative writing.

2. Recipient must be an active skier or snowboarder with a love for the outdoors, extreme sports and life in general.

3. Recipient exhibits good character, and is outgoing to both students and faculty alike.

The Ben Oberholtzer Memorial Scholarship application is separate. Form may be obtained in the Guidance Office.

39. OPEN LINK YOUNG VOLUNTEER RECOGNITION SCHOLARSHIP

A. Scholarship Amount: $500.00

B. Eligibility: 1. Graduating Upper Perk Senior;

2. Student is pursuing post-secondary academic or vocational education;

3. Has demonstrated consistent dedication to volunteering in the greater Upper Perkiomen Valley

over the course of his/her high school career;

4. Will use these monies in the upcoming school year.

5. Essay: 300-500 words, “How has volunteering helped you to define your goals for the future?”

This application is a separate form which may be obtained in the Guidance Office.

Deadline: Friday March 1st, 2019

40. THE PURPLE RIBBON AWARD

A. Scholarship Amount: $500.00

B. Eligibility: 1. Priority given to a student preparing for a career in social services, criminal justice or law enforcement.

2. Student has demonstrated a commitment or contribution to the health, safety, and well being of the community.

3. Essay: 300 word essay elaborating on community involvement, future goals and how those goals and /or community involvement may relate to working with people who have experienced domestic violence.

The Purple Ribbon Award application is separate. Form may be obtained in the Guidance Office.

41. NORMA REICHENBACH-NICHOLS SCHOLARSHIP

A. Scholarship Amount: $500

B. Eligibility: 1. Student is pursuing post-secondary training in Art, Art Education, or Education with

Preference given to students who attended Hereford Elementary

2. Minimum GPA of 3.0

3. Financial Need

4. Genuine love of working with children

The Norma Reichenbach-Nichols application is separate. Form may be obtained in the Guidance Office.

42. THE MIRIAM E. SAMPAYO NURSING SCHOLARSHIP

A. Scholarship amount: Approx. $1250.00

B. Eligibility:

1. Awarded to a female graduating student who will be pursuing a registered nursing program.

2. Minimum 3.0 GPA.

3. Must have shown volunteerism and leadership attributes in her/his community and school.

4. Essay – 300-word min./500-word max. on “How the student’s contributions will make a difference in the healthcare field”.

The Miriam E. Sampayo Nursing Scholarship application is separate and can be obtained in the Guidance Office.

Deadline: Friday March 1st, 2019

43. RUTH TREFFINGER MEMORIAL AWARD

A. Scholarship amount: Approx. $1250.00

B. Eligibility:

1. Awarded to a female student who will be continuing her education at the certificate, trade or associate degree level.

2. Minimum 2.5 GPA.

3. Essay – 300-word min./500-word max. on “How the student will apply her certificate or degree to contribute to her community/career development”.

The Ruth Treffinger Memorial application is separate and can be obtained in the Guidance Office.

44. UPPER PERKIOMEN VALLEY CHAMBER OF COMMERCE PERKUP EDUCATIONAL SCHOLARSHIP

A. Scholarship amount: $1,000.00

B. Eligibility: 1. UPHS graduating Senior pursuing a post-secondary academic or vocational education in a

business-related field;

2. Special consideration will be given for studies in STEM fields;

3. Residing in the Greater Upper Perkiomen Valley;

4. Maintaining above a “B” average;

5. Demonstrating leadership through participation in school and/or extracurricular activities;

6. Providing examples of active community service.

UP Valley Chamber of Commerce Scholarship application is separate. Form may be obtained in the Guidance Office.

Deadline: Friday March 1st, 2019

45. JOAN T SMITH SCHOLARSHIP AWARD

A. Scholarship Amount: $500.00

B. Eligibility: 1. Senior planning to attend Montgomery County Community College, with plans to eventually earn a 4 year B.A.

2. Student interested in the humanities, preference given to student interested in pursuing language.

3. 'B' average or higher.

4. Financial need.

The Joan T. Smith Scholarship application is maintained by Montgomery County Community College – students should visit Montco’s scholarship page for more info.

46. R. WINFIELD SMITH SCHOLARSHIP

A. Scholarship Amount: $500.00

B. Eligibility: 1. UPHS Senior planning on earning a Certificate or Associates degree from Montgomery County Community College.

2. Highly motivated student.

3. Financial Need.

4. 2.0 GPA or higher.

5. Must have attended at least 2 years at UPHS.
