This funding allowed me to travel to Paris, Grenoble, and Poland to speak with survivors and interview them. This honors thesis was submitted jointly to the Departments of History and French in 1993, and I graduated Magna Cum Laude, based on a combination of coursework and this thesis.2007-2010Global Women’s Health FellowshipGlobal Women’s Health Fellowship (Division of Women’s Health, BWH)PI/FellowThis fellowship funds gifted junior-level investigators pursuing research in global women’s health issues for travel to resource-limited settings as well as intensive training in research methodology at Harvard’s School of Public Health. Ultimately, this work will lead to a demonstration project to examine whether an evidence-based clinic intervention improves HPV vaccine initiation and completion rates overall and by gender, race/ethnicity, and insurance status.2015Systematic review on adolescent friendly HIV health servicesWorld Health Organization Contract(PI: Lisa Butler)My role as a scientific advisor on this grant is to assist Dr. Butler to review literature, summarize findings, and prepare a presentation for the World Health Organization Guidelines meeting. Current2012-2017HIV Treatment Refusal Among Adults Presenting for Testing in Soweto, South AfricaNIH K23 / 1K23MH097667PI ($921,846)My goal with this K23 award is to identify modifiable determinants of treatment-refusal, and design a targeted and informed intervention to counteract this phenomenon in Soweto, South Africa and prevent premature mortality in this vulnerable population. Using a mixed-methods approach, I will identify modifiable psycho-social determinants of antiretroviral therapy (ART)-refusal by performing in-depth qualitative interviews with 40 ART-eligible adults presenting for voluntary counseling and testing (VCT), and 10 healthcare providers, to inform category construction for a putative model based upon Anderson's Model of Healthcare Utilization. I will then examine correlates and outcomes of ART-refusal in 500 treatment-eligible adults, through survey implementation at the point of testing and for 6 months post-VCT. Finally, I will develop an intervention to promote HIV treatment acceptance among newly diagnosed HIV-infected individuals and pilot test it in a population of 100 treatment-eligible individuals.2015-2018 The Treatment Ambassador Program: Pilot testing a peer-driven intervention to increase treatment initiation among HIV-positive South AfricansNIH R34 / 1R34MH108393PI ($689,190)The research proposed here seeks to capitalize on our understanding of treatment refusal among people living with HIV in South Africa by designing a feasible and acceptable intervention to improve ART initiation. This multi-component intervention, titled the “Treatment Ambassador Program” (TAP), will target HIV+ people who do not initiate treatment within six months of learning they are eligible. It will be aimed at addressing barriers to ART initiation identified through our prior qualitative research, as framed through the Theory of Triadic Influence. As a physician-scientist, board-licensed in Internal Medicine and Infectious Diseases, I have focused my research over the past decade on the social determinants of health-seeking behavior among HIV-infected individuals in sub-Saharan Africa, and improving care for the most underserved. My work has been predominantly centered in South Africa, where our team identified the phenomenon of “treatment refusal” among individuals presenting for HIV testing. I am currently funded through a National Institute of Mental Health (NIMH) K23 Career Development Award (K23MH097667) to determine factors driving HIV treatment refusal among individuals presenting for voluntary counseling and testing in South Africa. In addition, I am the recipient of an NIMH R34 (R34MH108393) to develop and test a peer-based intervention, called “The Treatment Ambassador Program,” which focuses on bringing individuals living with HIV into care sooner, and a Burke Fellow through the Harvard Global Health Institute. I have written several perspectives for the New England Journal of Medicine (NEJM), where I served as an Editorial Fellow and am currently on the advisory committee. To date, I have completed or have in preparation over forty original research manuscripts, perspectives, and editorials, including publications in NEJM, Annals of Internal Medicine, Lancet HIV, and AIDS. Our research has also been featured in UNAIDS publications, and throughout the lay press including Scientific American and Reuters. I have presented our research at international conferences, including the International AIDS Society Conference (Durban, South Africa, 2016) and the International Conference on HIV Treatment and Prevention Adherence, as well as national conferences and NIH-sponsored symposiums. I was selected to join the Early Career Reviewer (ECR) program at the Center for Scientific Review (CSR), NIH, and to participate in a highly competitive program through the Office of Behavioral and Social Sciences Research at the NIH on Randomized Behavioral Clinical Trials. My research is informed by my training as a physician caring for individuals in underserved communities in Boston, with a focus on community-based strategies to improve outcomes in care. Two months of each year are devoted to direct patient care. During this time, I act as a clinical attending on the inpatient medicine service at BWH and Brigham and Women’s Faulkner Hospital. I also supervise and teach medical students from HMS, as well as house staff from BWH in bi-weekly hour long didactic sessions as well as provide direct guidance on oral presentation skills. In addition to my research and clinical work, I hold leadership roles at Harvard-wide programs, and provide mentorship for students, trainees, and junior faculty. I am the organizing founder of the Biannual Global Women’s Health Research Retreat, which I have run since its inception in 2011. This research retreat is an initiative designed to bring together junior investigators across all Harvard-affiliated hospitals and provide them with direct access to senior-level investigators involved in similar research. Through this network, I have fostered the development of over 30 junior investigators. In recognition of this effort, my Division Chief nominated me for a Pillar Award. Additionally, I directly mentor medical students (through the Scholars in Medicine program), residents, and fellows both through my research and my clinical work in the U.S. and South Africa. I have served as a Faculty of Epidemiology 208 through Harvard T.H. Chan School of Public Health Program in Clinical Effectiveness, and I also play an active leadership role as the former Faculty-Chair (2015-2016) of the Joint Committee on the Status of Women, through HMS. As a member of the Division of Medical Communications, I have been a member of the Perspectives Working Group at NEJM, and continue to publish articles in the lay press, including The New York Times, The Huffington Post, and Slate, and have provided commentary for National Public Radio on topics related to HIV and global health. ................
