Instructions for LTAC Provider Survey
The following information is provided to assist the respondent in answering the survey questions.
Provider Name: Enter name of Long Term Acute Care Hospital (LTACH)
Contact Name: Enter name of LTACH Contact submitting the survey questions
Contact Title: Enter title of the contact person identified in the “Contact Name” field
Contact Telephone Enter telephone number, including area code and extension, for the contact
Number: person identified in the “Contact Name” field
Please follow the detailed instructions to answer the various types of survey questions:
1. Questions that require only one answer to be chosen
If the question specifies ‘choose one’ or does not indicate “check all that apply” the respondent will choose only one answer option. Examples of this includes Yes/No or Yes/No/NA type of questions.
2. Questions that allow multiple answers to be chosen
If the question contains the directive “check all that apply”, then the respondent can choose as many answer options as apply.
3. Questions that include skip logic directives based on answer chosen
If an answer option is followed by a directive “Skip to #”, the respondent will not answer survey questions until reaching the question # identified in the “Skip to #” phrase.
4. Questions featuring “Other, specify:” as answer
If the respondent selects “Other, specify:” they are required to enter narrative detail to convey the answer to the question.
5. Questions that drive more questions based on response
In select cases when an answer is chosen, there may be additional parts of the question that will require the respondent to answer.
6. Questions with sub-sections that require the respondent to rank their responses
A sub-section of the question may require the respondent to identify and rank their top three responses in order of decreasing priority. Rank using the options provided in the question.
7. Questions that require narrative responses to part of or the entire survey question
The survey includes some descriptive or explanatory questions that require a narrative response. Please keep answers as detailed, yet concise as possible to convey a response. The online tool will indicate how many characters are allowed for the narrative.
