Drug/Dose

40 mg IV DAILY. Patient is able to take oral tablets. 40 mg IV DAILY ***FORMULARY for these patients*** 80mg IV Bolus followed by 8mg/hour for 72 hours post intervention or 80mg IV Q8H (double dose necessary for adequate treatment) for 72 hours post intervention. Interchange to Famotidine IV (Pepcid®) (No advantage of PPI over H2 antagonist) ................
