INDIVIDUAL SUPPORT PLANS POLICY AND PROCEDURES

POLICY STATEMENT

Each person supported by Community Living Windsor shall have an Individual Support Plan and planning meetings will be set up a minimum of once per year involving the person, their family, support staff and those people the person wants to help plan for their future. It is important that employees collaborate with and value the wishes of the people we support and others who are significant to them. If the person is a child and the Children's Aid Society is involved, a representative of the Children's Aid Society is to be invited to planning meetings. Such plans are to be developed based upon the needs, wishes and choices of the person.

PROCEDURES

1. Individual Support Plans are a formal agreement between the individual and Community Living Windsor (and possibly other service providers as well) indicating what supports the agency(s) is/are able to provide.

2. The Individual Support Plan shall identify the people involved, any short/long term goals, use of community resources, the funded services to be provided, any known resource allocation, any actions to be completed and who is responsible, any needed safeguards (including any assistance with financial management, assistance with banking) and dates for review of the plan.

3. In addition to the involvement of people closest to the person, Community Living Windsor will use other written resources such as the Provincial Application Form and any other helpful relevant assessments in the development of the Individual Support Plan.

4. Individual Support Plans will indicate:

the description of the support; responsibilities of people and agencies to provide support; expected outcomes; planning for and consideration of any associated risks.

6. A planning coordinator is identified at the beginning of the planning process. This individual is selected by the person and their respective family.

7. If the Planning Coordinator is a Community Living Windsor employee, that person will be responsible for:

having knowledge of or developing the contacts to access the potential services and

supports in the community that could assist in implementation of the plan;

ensuring the participation of everyone that needs to be involved in the planning process;

communicating with all involved people to ensure that a coordinated and informed process is maintained;

monitoring the expected outcomes identified in the Support Plan;

ensuring the planning takes place and is documented using the AIMS System, including the dates the plan was completed and any reviews as well as any changes to the plan and that appropriate signatures are obtained;

ensuring that Individual Support Plans are completed prior to supports commencing and are reviewed at least once per year (or as needed) at subsequent planning meetings.

8. The Individual Support Plan is to be completed prior to support commencing and then reviewed/updated at least annually. If the plan being developed is for a child, this is to be completed within the first 30 days of support. It is recommended that whenever possible that the Individual Support Plan be reviewed and completed in the birth month of the person.

9. Once completed, Quality assurance will upload the Individual Support Plan document in the AIMS system

10. The AIMS system will be used to monitor progress related to a person's goals and other areas of their support plan. The Manager, Support is responsible to ensure that progress is being documented and promoted.

11. The Manager, Support along with support from others will be responsible for:

ensuring that the ISP process is documented using the AIMS system.

ensuring that the Mission, Vision, Values statement is reviewed and the date is indicated on the Individual Support Plan.

ensuring that Communication and Personal Care Guidelines are completed prior to the planning meeting.

ensuring that the appropriate signatures are obtained on the Individual Support Plan.

discuss with the person with/or family/guardian how the information would potentially be shared with people outside the agency and ensure they are in agreement.

providing Individual Support Plan information to external agencies/personnel who are also involved in support with the consent of the person or their family/guardian.

ensuring the Review of Rights form has been signed and attached to the Individual Support Plan.

ensuring the Authorization to Release Information form has been signed and attached to the Individual Support Plan.

ensuring that the Individualized Abuse Prevention Training One-on-One Session form is

signed and attached to the Individual Support Plan.

ensuring that the "authorization to use photographic material" form is signed.

Signatures:

1. All people who are 16 years of age and over may sign their Individual Support Plan unless the person is a Ward of the Children's Aid Society, in which case the CAS signs on their behalf.

2. For children under the age of 16, the child's family or legal guardian may sign the Individual Support Plan.
