A helping hand towards greater

independence and community

integration for individuals with severe

and persistent mental illness.

Individual Support Services

are ¡°hands-on¡± individualized assistance with everyday activities that

are required by an individual with severe and persistent mental illness in

order to live independently in his/her community.

Eligibility: To Receive Individual Support Services A Person Must

¡¤ Be 18 years of age or older

¡¤ Be diagnosed with severe and persistent mental

illness

¡¤ Live either in a private home OR be transitioning to

independent housing from a licensed residential facility within

the next two months

¡¤ Need assistance with at least one Instrumental

Activity of Daily Living

Instrumental Activities of Daily Living:

¡¤ Meal Preparation

¡¤ Shopping for Household Necessities

¡¤ Using the Telephone

¡¤ Housecleaning & Laundry

¡¤ Money Management/Budgeting

¡¤ Transportation within the Community

¡¤ Medication Management

¡¤ Supervision & Cuing

Special Note: Individual Support Services are not available

to individuals living in licensed adult care homes or other communitybased residential settings, or who live in the home of and are supported

by a family member or other caregiver.

Individualized Service Plan

Individual Support is a one-on-one service provided directly to the individual.

This service is guided by an individual¡¯s unique Service Plan and must

reflect the strengths, needs and preferences of the person

receiving services.

Our Goal

A Helping Hand Along the Path Towards Independence

The goal of Individual Support Services is to provide

coaching to an individual with severe and persistent mental illness in the

daily life skills needed to live independently in his/her own home and

community. The individualized Service Plan includes a step-down plan

that identifies a person¡¯s natural support system (family, friends,

community resources, etc.). As a person transitions to these natural

supports or develops enough skill to live and function

independently, Individual Support for the person will be gradually phased

out until the service is no longer needed.
