Smoke Free Success!

Since adopting the Ottawa Model

Smoking Cessation Program in September 2013 our Family Health Team has….

What is a Smoking Cessation visit you ask?

The Smoking Cessation visit has been designed to assess patient’s smoking history and co-design a tailored plan for quitting with the patient. The plan includes setting the quit date, choosing a quit smoking medication, choosing a method of follow-up support, preparing for the quit date and planning to address cravings, withdrawal, and modifying routines.

What is so unique about our program is the ongoing support that is available. Nurses can provide you follow-up telephone support during those first difficult weeks.

Our current success rate for quitters who’ve signed up to the telephone support stands at __% smoke free after 30 days. The national average is between 4% and 7%.

Recognizing that it often takes 3 or 4 attempts to become totally smoke free we want those patients that are struggling or have relapsed to know we will always be here to support you.

Contact your doctor’s office to book a ‘Smoking Cessation’ visit today!

WHAT IS CHOLESTEROL? Cholesterol is a substance that is found in the blood. Everyone has some. It is needed for good health. The problem is, people sometimes have too much. Compared with people with normal cholesterol, people with high cholesterol have a higher risk of heart attacks, strokes, and other health problems. The higher your cholesterol, the higher your risk of these problems. You may hear your doctor or nurse talk about a few different types of cholesterol. Ask your doctor or nurse what your numbers should be. Different people need different goals. Having high cholesterol is just one of many things that can increase your risk of heart attacks and strokes. Other factors that increase your risk include: cigarette smoking, high blood pressure, and a family member who got heart disease at a young age. Your doctor will decide if you need medicine based on your age, family history, and other health concerns.
