Covid Intubation Training Drills – SJH Theatre TeamLow fidelity walk through of key issues when intubating someone who is symptomatic:Take Home Messages to get across:It will take longer to get everything done! That is unfortunately the nature of doing aerosol procedures whilst trying to avoid transmission. Planning: You want to try and avoid entering the room before you have the equipment/plan/people required, as getting extra kit that you don’t have is not that simple! This makes using a checklist and running through the plan beforehand particularly important.Personnel: Minimum number of people required to enter the room, with a runner in PPE outside. Not a training opportunity, so most skilled person to do the intubation.Procedure: Fairly standard intubation key points of difference to consider:Ventilating patient with mask- if you need to, do it with the best seal you canFirst attempt best attemptMake sure cuff is up after intubation BEFORE ventilation.Consider taping HME valve to ETT for transfer (if we intubate out of ITU, we will transfer on a C-Circuit)If transferring will do in PPE, with a runner ahead to open doors (ie team in PPE not to touch doors, etc)Use tube clamp to transfer from C circuit to ITU ventilator.Suggested Teaching PlanEquipment for teaching:Simulated airway trolley with equipmentMetal trolley to take into roomCOVID checklistCOVID action cardsMannikin to intubateClosed suction catheter mount to simulate going on the ventBlue tube clampsUnfortunately unlikely to have enough PPE equipment to use in training.Start outside room with airway trolley and metal trolley emphasising key points of Planning, Equipment, Personnel, demonstrating importance of checklist for making sure you don’t forget anything eg video laryngoscope, tube tie, drugs for ongoing sedation.Enter room with metal trolley and equipment and walk through intubation steps highlighting good seal, cuff up on ETT before bagging.Discuss transfer procedure and taping the HME valve for transferWalk through steps of clamping tube to put on ventEncourage spotter use for donning and doffingAuthor(s) E.MellanbyContributor(s) VersionEffective FromEffective To Change Summary ................
