COURSE REQUEST WORKSHEET JACKSON HIGH SCHOOL DISTRICT

RETURN TO THE MIDDLE SCHOOL GUIDANCE OFFICE DUE DATE FEBRUARY 14, 2020

GRADE 9

FIRST NAME___________________________

LAST NAME________________________________

Dear Parent/Guardian:

The courses listed on this worksheet were requested by your student with the help and advice of his/her counselor and teachers. These are tentative choices because:

1. Acceptance into Math, Biology, English, World Language and World History course levels will be predicated on the following: NJSLA assessments, writing skills, teacher recommendations and grades. Specific prerequisites can be found in the online Program of Studies located on the high school home page and Guidance homepage liberty or memorial.

2. PLEASE NOTE: Although we do our best to provide every student with every course he/she wishes to take, scheduling conflicts sometimes make this impossible and alternate course choices must be made. Course offerings may be closed due to budget constraints, teacher availability, or lack of student requests.

3. Core classes are placed by teacher recommendations.

We are sending this course request sheet home for your advisement and approval. If you wish any further clarification, please contact the middle school guidance office and set up an appointment with your son/daughter's middle school guidance counselor. If we do not hear from you, we will assume these requests may be processed for the upcoming school year.

PLEASE SIGN AND RETURN THIS SHEET TO YOUR CHILD'S COUNSELOR BY FEBRUARY 14, 2020
