Instructions for JCPS Education Records & Information ReleaseWhen to use this form:This form is for use in circumstances where the parent will still be the primary decision maker but would like to share access to information and records with another individual.This form can also be used by a student who is 18 or older to allow release of information or records to their parents.Examples:The parent would like for the student’s grandparent to be able to access his grades and information.The parent would like for the student’s step-parent to be able to view the information in Parent Portal and to speak to teachers regarding the student’s performance.The student is over 18, but he still wants his parents to be able to access his records.The parent wants to share access to the student’s records with a doctor’s office or a law firm.Who completes this form: This form should be completed by a parent who wants to release information or records to another person, agency, or entity. NOTE: Parents can only share the rights they have. If a parent does not have the right to view the student’s records, he/she cannot share the right to view records.Parent Portal:If the individual needs access to Parent Portal, for example, a step-parent, the parent should check the box regarding Parent Portal so the individual can have their own Parent Portal account. Parents should not share their Parent Portal log in information with other individuals. If the parent is allowing access to Parent Portal, please emphasize to them that this access will allow the individual to apply for school placement and transfer and will allow the individual to check the student out during the school day and after school and will allow the individual to sign permission slips for field trips and extracurricular activities.If the release provides rights to an entity or agency, it is not appropriate to check the Parent Portal box. Examples of entities or agencies include: a physician’s office, Centerstone, an attorney or law firm.How to record this form in Infinite Campus:When you receive a signed release for an individual, add the individual in Infinite Campus as a non-household relationship, unless the individual lives in the same household as the parent. If the parent has marked the Parent Portal access box, check the Guardian and Portal boxes in Infinite Campus. If the Parent Portal access box is not marked, check only the Guardian box.Do not change the rights of either parent when entering information from this form into Infinite Campus.Do not add an entity or agency as a non-household relationship in Infinite Campus.Forward a copy of this form to ECE Records if the student is an ECE student.Revocation:This form can be revoked in writing at any time by the parent who signed it. A revocation form is provided on the back of the form for convenience.If the parent revokes Parent Portal rights, you need to revoke the Parent Portal access.Revocation forms need to be included in the student’s cumulative file (VISI) and sent to ECE records.Instructions for JCPS Power of AttorneyWhen to use this form:Use this form when The child will be living with someone other than their parents AND That person will be making the student’s educational and medical decisions AND The parent(s) still has(have) guardianship of the child ANDThe child is under 18This form does not allow the Attorney in Fact (the person the child will be living with) to attend ARC meetings or make ECE decisions. Please contact ECE if the parent needs to appoint someone to attend ARC meetings on their behalf.Who completes this form: This form should be completed by both the parent and the person the child will be living with (called the “Attorney in Fact”). Student Residency:This form should only be used in situations where the student will be physically living with the Attorney in Fact. When providing this form to parents, emphasize that it will allow the individual to apply for school placement and transfer and will allow the individual to check the student out during the school day and after school. Also emphasize that in the event of an emergency, JCPS will be calling the Attorney in Fact first.If the power of attorney is not signed by the Attorney in Fact, JCPS will not honor it for student assignment purposes.How to record this form in Infinite Campus:When you receive a completed Power of Attorney form, create a new household in Infinite Campus for the Attorney in Fact, list them as “Power of Attorney” for the relationship, and mark the Guardian and Portal boxes. The Attorney in Fact will become the student’s primary household. The parents’ household will become the secondary household. All relationships will remain the same. Do NOT end date the parents’ relationships or uncheck the parents’ Guardian and Portal boxes. A Power of Attorney does not terminate the rights of the parents.Forward a copy of this form to ECE Records if the student is an ECE student.Revocation:This form can be revoked in writing at any time by the parent who signed it. A revocation form is provided on the back of the form for convenience.This form can also be revoked in writing by the “Attorney in Fact” (adult with whom the student was living). When this form is revoked, the student’s residence in Infinite Campus reverts to its previous state. If you receive a revocation, you need to revoke the Parent Portal access.Revocation forms need to be included in the student’s cumulative file (VISI) and sent to ECE records.Other Power of Attorney Forms:JCPS does not recognize other Power of Attorney forms for residency purposes.JCPS does accept the Military Power of Attorney form. Instructions for JCPS Caregiver AffidavitWhen to use this form:Use this form when a minor student will be staying with an adult relative who is not the student’s parent, de facto custodian, guardian, or legal custodian AND the student’s parent or guardian is not available to complete a Power of Attorney form. Note: This form can only be used by adult relatives of the student.Note: The student must be under 18. This form is not valid if the student is 18 or older.Who completes this form: This form should be completed by the Caregiver (the adult relative the student will be staying with).Other Requirements:The Caregiver must provide a reason why the parent or guardian is unavailable.The Caregiver must also provide a description of the efforts made to locate the parent or guardian.All fields must be completed and all lines initialed or signed, as appropriate.This form is only valid for one year and must be recompleted each year.If the student stops living with the Caregiver, the Caregiver must notify JCPS. The Caregiver Affidavit is not valid if the student does not live with the Caregiver. Forward a copy of this form to ECE Records if the student is an ECE student.How to record this form in Infinite Campus:When you receive a completed Caregiver Affidavit, create a new household in Infinite Campus for the Caregiver, list them as “Caregiver” for the relationship, and mark the Guardian and Portal boxes. Add an end date to the Caregiver relationship of 1 year from the date of the Caregiver AffidavitThe Caregiver will become the student’s primary household. The parents’ household will become the secondary household. All relationships will remain the same. Do NOT end date the parents’ relationships or uncheck the parents’ Guardian and Portal boxes. A Caregiver Affidavit does not terminate the rights of the parents.Forward a copy of this form to ECE Records if the student is an ECE student.Revocation:This form can be revoked in writing at any time by the parent or the Caregiver who signed it. A revocation form is provided on the back of the form for convenience.This form is also automatically revoked if the student turns 18 or no longer lives with the Caregiver. When this form is revoked, you will need updated contact information from the student.If you receive a revocation, you need to revoke the Parent Portal access.Revocation forms need to be included in the student’s cumulative file (VISI) and sent to ECE records. ................
