STATEMENT OF WORKNorth Texas VA Medical Center4500 S. Lancaster RoadDallas, Texas 75216Casework for Exam Room Renovations549-18-1-040-0001 GENERAL SCOPEThe Veterans Health Administration (VHA) has a requirement for HVA Systems for North Texas VA Medical Center that meets the VHA specifications and high quality threshold while integrating excellent environmental stewardship. All products and services shall be on the General Services Administration’s (GSA) current Federal Supply Schedule (FSS) Schedule 71 or 71 II K contract.The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. The contractor shall provide all labor, layouts, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. *In this SOW, the terms “Contractor,” “Vendor,” and “Winning Bidder” are used interchangably.*The plan is renovate two Medication Rooms (5A109E and 5A124R), two Nourishment Centers (5A109G and 5A124U), Podiatry Procedure Room (3A338), and updating other exam rooms in the clinics which will include the installation of modular casework. PERFORMANCE REQUIREMENTSManufacturers should grade in highest quality finishes if not stated in the specifications, or allow for COM, which shall be purchased by the manufacturer. If there are anti-microbial finishes grade those in where available (counter-tops, handles, etc…). All drawings, finishes, and specifications should be selected, approved and confirmed by VA designer prior to manufacturing.The vendor shall provide and be responsible for the technical assistance, development and generation for final specifications.These units will be installed in six to eight (6-8) different stages. After award, the winning bidder will need to coordinate with the COR and/or VA Designer to plan out each phase of installation. Duty hours will be between 8:00AM and 4:30PM. The Government recognized US holidays are: New Year’s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. Installations times and dates will be coordinated with the VA Designer and may occur outside of regular duty hours (weekends, late evenings, etc…) to minimize impact on patient care. The contractor shall: Submit shop drawings of the proposed installation, and receive VA approval (allow the VA 7 days for approval), before starting installation. If the contractor deems the 90 day and 60 day time frames to be unreasonable, the contractor shall submit a proposed schedule with their price proposal.Coordinate and provide project management of all products and services to manage, design, order, ship, and deliver new products from the manufacturer until final acceptance by the Contracting Officer Representative (COR)/ VA Point of Contact. Provide maintenance manuals for any and all products provided.PRODUCT SPECFICATIONSPART 1 - GENERAL 1.1 SUMMARY Work Included: Provide labor, materials and equipment necessary to complete the work of this Section, including but not limited to the following: Modular metal casework including floor base units, tall units, sinks and faucets, desks, mobile units, overhead units, specialty units, countertops, and accessory items. 1.2 SYSTEM DESCRIPTION Design Requirements: Casework shall be metal, modular, with each unit being an individual, interchangeable, integral part of assembly for making up desired casework unit. Each sectional unit shall be rigid and depend on no other component part of complete assembly for its rigidity. Internal components such as drawers, glides, shelves, and similar items shall be modular and interchangeable. Performance Requirements: Provide healthcare casework which has been manufactured, fabricated and installed to withstand the medical environment and maintain manufacturer's warranted performance without defects, damage, or failure. 1.3 SUBMITTALS Product Data: Submit manufacturer’s product data for each type of product. Shop Drawings: Submit shop drawings including the following: Complete list of products, sizes and manufacturer’s descriptive and technical literature. Requirements for utility connections. Requirements for blocking and support. Keying schedule as applicable. Samples for Verification: Submit representative sample of each type of finish and color. LEED Projects: For projects seeking LEED certification, submit manufacturer’s documentation for the following:LEED IEQ 4 compliant Composite wood. Recycled content of metal, wood, and plastics. Water use reduction based on qualified plumbing fixtures. Installer Reports: Submit copy of installer’s reports for the following: Certification that installer is acceptable to the manufacturer. Manufacturer’s field service report. Pre-installation meeting report. Operations and Maintenance Data: Submit manufacturer’s recommendations for cleaning and maintenance, including a list of spare parts and information for ordering. Warranty: Submit executed copy of manufacturer’s standard limited warranty. 1.4 QUALITY ASSURANCE Manufacturer: Regularly engaged in manufacturing healthcare casework, ISO 13485 and ISO 9001 certified. Installer: Minimum 2 year documented history of installing similar casework and acceptable to the manufacturer. Installer shall accept responsibility for all field verifications. Pre-Installation Meeting: Convene a pre-installation meeting prior to installation to review existing conditions, schedule, sequence of installation, and proper placement of utilities. 1.5 DELIVERY, STORAGE AND HANDLING Deliver and store casework as recommended by manufacturer. Protect from the weather, humidity, temperature variation, dirt, dust, or other contaminants. WARRANTY Modular Casework Limited Warranty: Provide manufacturer’s standard limited warranty for the items listed below, with additional exclusions as included in manufacturer’s standard warranty. Furnish copies of warranties for components not manufactured by Midmark Corporation but incorporated into the casework as applicable. Door and drawer fronts, casters, 5 years. Task lights, 3 years. Laminate top surfaces and accessories, 1 year. PART 3 - EXECUTION 3.1 EXAMINATION Examine substrates and site conditions for compliance with requirements for installation tolerances and other conditions affecting performance. Proceed with installation only after unsatisfactory conditions have been corrected. 3.2 INSTALLATION Install products in strict accordance with manufacturer’s instructions, approved submittals and in proper alignment with adjacent work. Provide installation method suitable for substrate and project conditions. Test operable components for proper operation and adjust until satisfactory results are obtained. 3.3 CLEANING AND PROTECTION Clean exposed surfaces using methods acceptable to the manufacturer which will not damage finish. Protect casework against damage until accepted. Restore damaged components and finishes as necessary so no evidence remains of corrective work. Written approval from the VA POC must be received before proceeding with the work. The request shall be submitted at least (5) five work days prior to the proposed performance of work, and it shall indicate the day (s), amount of time of work, and reason why performance is necessary during nonstandard hours.The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission. Contractor shall comply with local security policies and procedures and mission statements, during all site visits on Government property.The Contractor personnel shall present a neat appearance and be easily recognized as contractor employee. A VA provided badge must be worn at all times while on government property and must be worn above the waist and must be legible to read by other. This badge will be issued at the Engineering Building front desk, Building 9.The Contractor shall ensure policies and procedures established that protect the safety and welfare of patients, employees, and the community to minimize or eliminate safety or environmental threats.The contractor shall be knowledgeable of and shall comply with all applicable federal, state, and local laws, Building Codes, Life Safety Codes, ADA/ADAG, permits, VA Design Standards (cfm.til/dguide.asp), and instructions required for the performance of the duties specified.The contractor is responsible for ensuring that its firm and staff maintain any generally required professional certification, accreditation, license, bond, and proficiency relative to their area of expertise. The Contractor shall retain documentation of such records. Cost of installation and travel of replacement material and parts shall be borne by the Contractor/ Vendor.The products will be free from defects in workmanship and materials for a period of at least one year. Shipping Instructions: Items shall be shipped directly to the warehouse at North Texas VA Medical Center at 4500 S. Lancaster Road, Dallas, Texas 75216, unless otherwise directed by the COR or designated VA POC.These units will be installed in six to eight (6-8) different stages. After award, the winning bidder will need to coordinate with the COR and/or VA Designer to plan out each phase of installation. The contractor will coordinate all deliveries and installation times with the VA COR or designated VA POC.All shipping and handling charges shall be paid by the contractor. Responsibility and Liability for loss or damage shall remain with the contractor until final inspection and acceptance by the VA. Inspection and Acceptance Criteria: Contractor shall receive all products, complete inspections and perform any damage reports as necessary. All finished surfaces shall be free of scratches, mars, dents, or blemishes, and withstand staining, and exhibit to flaking, cracking, or loss or adhesion. Furnishings shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing.Period of Performance: These units will be installed in six to eight (6-8) different stages. After award, the winning bidder will need to coordinate with the COR and/or VA Designer to plan out each phase of installation.The date of shipment(s) shall be scheduled between the bid winner and the VA after the placement of the PO to the bid winner. The Vendor shall track manufacturing schedule and notify the COR and VA Interior Designer with updates of estimated completion date by email.Delivery, Storage and Handling of Materials: Contractor shall place all orders, track orders, schedule deliveries and coordinate trucks with the manufacturer and delivery according to VA project schedule.The product must not be placed into production without clearance from the VA POC. Contractor shall contact the POC at least 14 working days prior to start of work/ shipping to ensure that the space is ready for install.In event there is a delay in delivery; Contractor must notify the VA POC immediately.Contractor shall provide the VA POC the scheduled date and time that the delivery truck will be arriving at least 48 hour ahead of time. Contractor must ensure that the driver of the delivery vehicle has copies of the delivery order and shall not attempt to deliver any item before the scheduled delivery date.Contractor must cooperate with VA Logistics to properly receive all products and furnishings at VA facility. Product shall be factory packed with adequate protection to all sides and corners to prevent transit and handling damage. The Contractor/ Vendor shall provide (if needed) 45 days free storage in a conditioned facility that meets all building requirements including but not limited to sprinkler, security, safety, temperature and environmental controls- as not to void warranty.The Contractor must notify VA of specific location of any and stored parts.Receiving and Unloading:Contractor shall place all orders, track orders, schedule deliveries and coordinate trucks with the manufacturer and delivery according to VA project schedule.Contractor shall receive all products, complete inspections and perform any damage reports as necessary and notify the VA POC.If the Contractor is unable to complete the assembly and placement of all unloaded items before the end of the work day, the Contractor will be responsible for moving these items to a secure location until the next available work day. The Contractor will be responsible for moving the items from the overnight storage site to its designated position.Contractor shall store damaged items in the location designated by the POC. The Contractor shall maintain a complete file of all documents relating to each discrepancy and copies of all Discrepancy Reports shall be forwarded to the POC on a daily basis. Contractor shall repair or coordinate for replacement of damaged, defective, or missing items.Protection of Property:Contractor shall protect all items from damage. The contractor shall take precaution against damage to buildings, grounds, and furnishings. The contractor shall repair or replace any items related to buildings or grounds damaged accidentally, or on purpose due to actions by the Contractor, utilizing materials of the same quality, size, grade and color, to match existing work. Contractor shall be prepared to provide floor protection while working in VA facilities. All material handling equipment shall have rubber wheels to help prevent damage to the floor.Installation: Contractor shall be responsible for following all safety and security guidelines within the area in which work is being performed, to include securing and contractor owned tools and/ or equipment, and any on-site storage being utilized.The contractor shall install all equipment in accordance with each requirement statement and basic component floorplan, designed and provided by contractor.Contractor shall uncrate ALL items, receive, and perform all required assembly in accordance with the manufactures’ instructions.Contractor shall prevent their personnel from entering any area other than the designated work area.Contractor shall maintain a means of egress within all designated work areas to comply with fire codes.Contractor shall inspect all products to ensure that the products is free of surface dirt, clean and polished, free of defects, and that the installation is complete and ready for use.Contractor shall do a final walk-through with VA POC and provide a punch list before releasing crew for the day.Quality Control:Contractor will be responsible for the removal and disposal of all trash/ debris connected with uncrating and assembling chair and other items installed under this contract. Final acceptance from the contractor will not occur until ALL debris connected with installation is removed from the VA site.Recyclable cardboard products shall be disposed of in accordance with applicable statues, in respective containers. The Contractor is responsible for providing trash containers at an offsite facility.Contractor is to recycle as much as possible and report weight land-filled and weight recycled with progress payment requests.Packing materials will not be stored in the buildings for any period exceeding 24 hours.The Contractor shall ensure their personal eat, drink, and/or smoke only in designated areas. Personal trash (food wrappers, drink bottles, etc...) shall be removed from the site on a DAILY basis by the Contractor. ................
