Pediatric Patient Education InitiativeUnits: 9S, 10STargeted Patients on the following medications: New initiation of targeted medicationsImmunosuppressantWarfarin/other non-IV anticoagulantsOral chemotherapyEnoxaparin (prophylaxis and treatment), new and continuation of therapyLong-acting/maintenance inhaler, new and continuation of therapyEducation will be provided on all discharge medications, not just those listed aboveIdentification of patients to be completed by: Point-of-care pharmacist (PCP) by review of daily census on unit and/or “EXP Disch Date” in EPIC header, and by Clinical Pharmacy Specialist (CPS) while on rounds. Proposed Workflow: Monday – Friday only. Education may be provided between 8:30-4. PCP will be unavailable between 11-12:30 due to lunch coverage. ~8:30- PCP will review census on 9S and 10S to review planned discharges for unit, noting those for both that day as well as later in the week.Education can be provided on the day of discharge, but may also be done earlier as appropriate.PCP will compare discharge list with patient profiles to determine which patients meet the targeted criteria.PCP will coordinate with CPS to determine who will provide education to which patient. PCP will notify RN via page to Ascom after education has been provided.Document education via Patient Education Activity, Excel tracking document (“+Patient Education Log”, found at I:\+Pediatric Pharmacy\Patient Education), and i-Vents/Progress Notes where needed. [see separate document for documentation information] Patient Education Column will be used to communicate between pharmacists, and may include:Predicted date of discharge, especially if in futureDate/time of planned education, if later in the weekSpecific time when family/caregiver may be present for educationSpecial education requests by nursingNeed for interpreter*Use date format (MM/DD) at the beginning of documentation in this column, much like we do for the Rx HandoffOther considerations:Goal to provide education to at least 2 patients per day, but will likely not reach all patients that meet criteria during initial rollout. South queue coverage will need to be provided by coordinators, North PCP, and/or KKI PCP during patient education Points to focus on: purpose of medication, how to take, and side effects. Oral syringes: do not provide oral syringes to patients/families, as we do not want to cause confusion with what may be dispensed by the outpatient pharmacy. However, how to use and clean a syringe may be helpful teaching points.Additional Resources:Custom-made handouts are not permitted. Patient education information from Lexicomp, Krames (accessible via EPIC, under Patient Education), and from relevant policies in HPO may be given to the patient/family.
