Scholarship Application

Scholarship Description:

Miller Heights Elementary PTO will offer one $500 scholarship each school year. To be eligible to apply and receive this scholarship, you must meet the following criteria:

1. Be a graduating senior of Liberty or Freedom High School and have attended Miller Heights Elementary School for a minimum of three (3) years.

2. Have a minimum GPA of 3.0 through his/her high school career. 3. Be attending an accredited college, university, vocational, or technical school

in the Fall of his/her graduating year.

Additional Items:

The applicant must include the following items with his/her application:

1. A copy of his/her high school transcript. 2. A letter of recommendation from a high school teacher. 3. A letter of recommendation from a community/school leader (coach,

instructor, guidance counselor, scout leader, employer, etc.)

Application Procedure:

1. Applicants should complete the attached application along with the "additional items" above addressed to: Miller Heights Elementary School, Attention: Principal, 3605 Allen Street, Bethlehem, PA 18020.

2. Applications must be received no later than April 30, 2021. 3. No late and/or incomplete applications will be considered. 4. The decision made by the selection committee is final.

Awarding the Scholarship:

1. Miller Heights' Principal will notify the recipient by June 1st. 2. Proof of acceptance will be required to an accredited college, university,

vocational, or technical school. 3. After receipt of proof of acceptance, a $500 check will be made payable

directly to the higher education institution.
