Health Forms (2021-2022)

Annual Physical Forms Health exams forms (form CH205 from NYC DOH) signed and stamped by a physician must be obtained and given to the nurse's office annually. Health exam forms expire one year after the date of the physical exam.

Immunizations In compliance with NYS Public Health Law Section 2164, immunizations are required for all students to attend school in accordance with the ACIP-recommended schedule. Children must be excluded from the school if they don't meet immunization requirements for their academic grade or did not provide documentation of immunization within first 14 days of attending the school. Students in process of catching up with off-schedule immunizations or whose vaccines become due during the school year must receive all vaccinations within 14 days of becoming due or they will be excluded from school. Alternative vaccine schedules or spacing are not accepted in accordance with NYS Public Health Law. Note, mandatory immunizations do not currently include the COVID-19 vaccine.

Medical exemptions are only permitted if there is a documented medical condition that precludes specific vaccinations. Medical exemption forms must be completed annually for each vaccine for which an exemption is being requested, and will be reviewed by the NYC Department of Health for final approval.

Medical Exclusion from School Due to COVID-19 If your child has any of the following symptoms of COVID-19, they must stay home from school and be evaluated by a medical provider. Students must have a negative COVID-19 test result, no fever (100.0F?) for 24 hours without the use of fever reducing medication and written medical clearance to return to school after experiencing COVID-like symptoms:

If a student is diagnosed with COVID-19 or develops symptoms and is not tested for COVID-19, they must isolate at home and may not attend school for 10 days from the date of symptom onset and have no fever (100.0F?) for 24 hours without the use of fever reducing medication and symptom improvement in order to return to school.
