How it works

You will receive $225 each quarter to buy covered over-the-counter (OTC) products. ? At the start of each quarter, your OTC allowance will be added to your mail-order account. Any unused dollars will rollover to the next quarter but must be used in full by December 31, 2021. ? You can order products from this mail-order catalog online, by phone, or by mail. ? The products listed in this catalog are in alphabetical order under specific covered item categories. To order, you will need to provide the product name, item number (SKU), and quantity you would like to order. ? Products will be shipped directly to your home. Please allow up to 7-10 business days for delivery.

Important things to remember:

? Keep this catalog and use it to order products from January 1 through December 31, 2021. ? To take advantage of this allowance, you must order products listed in this catalog. ? Your order total may not exceed your available allowance amount. Cash, checks, credit cards,

or money orders cannot be accepted. ? Because of the personal nature of these products, returns cannot be accepted. ? Your order total will be applied to the quarter in which the order is received. ? Some limitations and restrictions may apply for covered products. ? Make sure to talk to your doctor before taking any OTC medications. ? Covered OTC products are for members only. Members cannot purchase products for family

members and friends. ? The products included in this catalog are offered through a third-party company called

DrugSource, which is responsible for processing your orders and shipping your products to you as quickly as possible. ? There is no charge for shipping and handling. Any additional costs you may notice are to ensure you receive the best possible customer experience and that your orders arrive accurately and timely. ? The product prices included in this catalog are set by DrugSource and may be subject to change.

How to place an order

Order online. Go to shopping.upmcsnp. You can look through the catalog, view your account balance, and buy covered products. You will need your UPMC for Life member ID card and date of birth to log in. Order by phone. Call DrugSource at 1-877-418-8821 (TTY: 711) Monday through Friday from 9:30 a.m. to 11 p.m. to place your order over the phone and have your questions answered by the experts. Be ready to provide your UPMC for Life member ID number and date of birth. Order by mail. Complete and mail an order form to the address below. Be sure to include postage.
