As a member of UnitedHealthcare?, you get a Health & Wellness Products Catalog benefit. You will receive credits each quarter, on the first day of January, April, July and October. Refer to the enclosed letter with this catalog or go to HWP for credit expiration details. You can use your credits to purchase approved over-the-counter (OTC) products and have them delivered to your home.

1 Follow the instructions on your card to activate it before using it by calling 1-833-832-7306, TTY 711.

2 Use your 17-digit account number and 4-digit security code when ordering approved OTC products.

You choose how to buy products

This catalog includes a product list that makes it easy to order approved OTC products you may need. It includes hundreds of popular products that you can buy and have them delivered to your home.

To get started, use this catalog or go to HWP to find the products you want to buy.

Online Go to HWP and add products to your cart on the Order page.

1 Sign in to HWP using your Easy &

HealthSafe ID?.

Fast

If you don't have a HealthSafe ID?, click Register Now to create one. You'll need your member ID card and a valid email address to get started.

2 Once signed in, click the Order OTC Products button, then click the Ready to Order button to view products.

3 Add items to your cart by clicking the Add to Cart button in the product details.

4 When you're done adding items, click Cart in the upper right to review your order.*

5 To complete your order, click the Check Out button and enter your shipping details. Then click the Place Order button to receive your products with fast delivery from Walmart.

* You'll see the amount of credits used for your purchase. If your total is more than your current credit balance, you will need to remove items from your cart.

By phone 1. Make note of which products you want to buy from the catalog before calling. The

customer service representative will need to know: ? Your 17-digit account number and 4-digit security code. ? Your name and mailing address. 2. Place orders by calling 1-833-845-8798, TTY 711, 8 a.m. ? 8 p.m., local time, 7 days a week, October ? March; Monday ? Saturday, April ? September.

By mail 1. On the order form included in your welcome packet, list the product items in order of

preference. 2. Confirm your order form includes the following information:

? Your health plan member ID number ? Your full name ? Shipping address and ZIP code ? Daytime phone number 3. Insert the completed order form in one of the postage-paid envelopes provided. 4. Mail your form to: Healthy Benefits Plus PO Box 46220 Plymouth, MN 55446-9907

For mail-in orders, if the total is more than your current credit balance, items will be removed. The last item listed will be removed from the order until the total is under your credit balance.
