Re: Invitation to submit Management and Operations Profile

Dear Mr. Brayton,

Xiphias Wellness, Inc. is invited to submit a Management and Operations Profile (Application 2 of2) to the Department of Public Health ("Department").

On or before December 11, 2017, the Department must receive the Management and Operations Profile, along with all required attachments, as outlined in the Management and Operations Profile instructions.
