L.A. Care Housing Initiatives

We currently have two housing initiatives for L.A. Care members who are experiencing

homelessness and are high utilizers of health care services:

2017--Present: L.A. Care's Community Health Investment Fund (CHIF) grant with L.A. County Department of Health Services

Housing for Health (HFH) and Brilliant Corners

2020--Present: L.A. Care and HFH's Housing for Healthy CA program

Program Eligibility

L.A. Care CHIF Grant

Both L.A. Care members (including Plan Partners) and non-

members

Must be experiencing homelessness & high acuity or high utilizer of

health services

Housing for a Healthy California

L.A. Care members only (excluding Plan Partners)

Must be experiencing homelessness & high acuity or high utilizer of

health services

L.A. Care, Housing for Health, and Brilliant Corners

In 2017, L.A. Care made a $20 million Community Health Investment Fund grant to house 300 people experiencing homelessness over five years in partnership with HFH and Brilliant Corners, a nonprofit housing agency.

The grant has already housed over 300 households and achieved a 90% one-year retention rate for those housed through the program.
