Geographical Area Home based/Wiltshire territory

Hours of Work 37 hours per week Monday to Friday

Grade £35,000 to £40,000 depending upon experience

Contract Jan 4th – April 30th 2021 in the first instance

Purpose of the Post

To oversee the setting up of the new Hospital Liaison Service.

To ensure Hospital Liaison Service meets agreed deliverables, and is co-ordinating well with all relevant hospital and Adult Social Care teams and in-house services.

To support and collaborate with other Senior Managers in Carer Support Wiltshire in the management of services delivered by Carer Support Wiltshire.

Key duties and responsibilities:

To be responsible for the new Hospital Liaison Service,:

1. To support and guide the Hospital Liaison Service Lead in setting up the Service, using a project management methodology

2. To oversee the recruitment and induction of a new Hospital Liaison Service staff team

3. To ensure the service is fully co-ordinating with relevant hospital services, including the Flowhub, with Adult Social Care teams, and with in-house and other external carers services and partners.

4. To ensure service delivery is in line with Carer Support Wiltshire (CSW) policies and procedures, and if needed, to revise and/or formulate new policies and procedures; and promote their implementation

5. To hold regular team meetings to share good practice, to identify and resolve problem areas, and to ensure good communication

6. To build positive and effective working relationships with all key partners

7. Evaluate overall service performance by gathering, analysing and interpreting data and metrics

8. To manage all reporting on progress on deliverables to commissioners at Wiltshire Council and in CSW

9. To represent and promote the Hospital Liaison Service and Carer Support Wiltshire to senior staff and key contacts within the hospitals, Wiltshire Council, BSW Clinical Commissioning Group.

10. To manage the budget for the service, review financial performance information and adjust operational budgets as needed

Additional Responsibilities

As a member of the Senior Management Team, the post holder will be expected as necessary to support colleagues by:

To work collaboratively with SMT colleagues on service delivery and management activities, taking part in SMT meetings, contributing ideas, expertise, knowledge and enthusiasm

Participate in reporting via the CEO to the Board of Trustees on own areas of responsibility

Actively contribute and participate in promotional activities, and represent Carer Support Wiltshire in a positive manner, and to raise awareness of carers’ needs.
