Re: CCGPP Progress Report
At this time I would like to provide you the progress report on the current activities of the CCGPP that was recently presented at the COCSA annual meetings in San Antonio earlier this month.
As you know the CCGPP continues to pursue its initial project of developing literature syntheses on the various anatomic regions defined in our initial project. Those literature syntheses are progressing as anticipated and all are nearing completion. Currently, our work products to date are posted on the internet at for public review.
The chapter on the thoracic spine is currently nearing completion and is hoped to be posted in the coming months. After substantive literature review, the cervical spine team has elected to adopt new guidelines recently released and based on the latest available literature that appeared in a dedicated issue of Spine. They will also be presenting their report in the next few months. A report addressing diagnostic imaging guidelines is also scheduled to be released in the near future.
The CCGPP did complete its Delphi consensus process regarding chiropractic low back care. Its report, “Chiropractic Management of Low Back Conditions, along with CCGPP’s original low back literature synthesis has been subsequently referenced in the Occupational Disability Guidelines (ODG), resulting in substantive changes to that document.
As a landmark event, Dr. Hawk, CCGPP Research Commission Chair, has arranged to have the completed literature syntheses chapters published in the Journal of Manipulation and Physiological Therapeutics (JMPT). The November/December 2008 and January 2009 issues will be dedicated to publishing those literature synthesis chapters, which will make them available in the worldwide database of searchable, citable health care literature for use in the support and defense of appropriate chiropractic care.
The Rapid Response Team also continues to be active. Currently, they are assisting the ACA Insurance Committee in addressing the Milliman Guidelines as they apply to chiropractic care. Meetings have been held and the team is currently developing information to enhance chiropractic coverage in that document. As we did with United Health Care, the CCGPP’s Rapid Response Team is also attempting to revise Tufts Health Plan determination to deny coverage for chiropractic care of pediatric patients. To date, our requests have been declined. However, we continue to search for the additional evidence necessary to overturn those policies, as CCGPP feels they are not based on available and accurate evidence. Therefore, Dr. Hawk is leading a team, at the behest of FCER, to conduct a Delphi consensus process on chiropractic pediatric care.
The Rapid Response Team also has additional requests to consider and/or are being worked on currently. One example is that CCGPP has been contacted directly by the New York State Chiropractic Association (NYSCA) for assistance in developing guidelines for utilization in their workers compensation program. They are looking for logistical direction to accomplish this goal, based on our success in a similar process in the California’s workers compensation system. This is a boding trend that is continuing to arise in various states across the nation. The CCGPP is currently discussing this request with the NYSCA.
CCGPP is also currently initiating another Delphi consensus project for the ACA to define levels of chiropractic care relative to the greater health care arena. These levels include acute care, chronic/recurrent care, maintenance care and health promotion/wellness care. This is to be a multidisciplinary project and we are currently accepting nominees for our expert panel reviewers. After considerable discussion at our annual meeting, the CCGPP Council recommended that we ask the team leads to help identify areas of significant evidence weakness in their respective documents. The two-fold purpose of this exercise is 1) to conduct additional relevant consensus documents to augment the current existing evidence and 2) to identify those areas most frequently encountered in daily practice, conduct a profession-wide survey of those areas to prioritize them and then make that information available to the research community for consideration as topics of new original research.
