MINNESOTA MORTGAGE LOAN ORIGINATOR LICENSE
This document includes instructions for an individual new application request. If you need to complete an application for a company or branch location, refer to the appropriate new application checklists.
Total License costs: $120 including the NMLS processing fee. A $15 fee for a credit report will be added if one has not been authorized through NMLS in the past 30 days. You will also pay an additional $36.25 if you authorize a criminal background check at time of application. Fees collected through the NMLS ARE NOT REFUNDABLE.
A sponsorship request must be submitted by your employer before your application request will be approved. You are not authorized to originate residential mortgage loans involving Minnesota dwellings unless in an "Approved" status.
Use the checklist below to complete the requirements for the Minnesota Department of Commerce (DOC). The checklist provides instructions and requirements for information to be entered in NMLS, the documents that must be uploaded into NMLS, as well as the documents that must be sent outside NMLS.

Criminal Background Check: Authorization for a FBI criminal history
background check to be completed in NMLS. Pursuant to MN Statute
58A.05 (1) and (2), if applicable, provide an explanation as to the
circumstances surrounding any criminal background and include any
applicable legal documents with the criminal complaint(s), sentencing
and/or disposition. If charges were dropped include proof of dismissal.

Credit Report: Authorization for a credit report must be completed in
NMLS. Pursuant to MN Statute 58A.05 (3), the Commissioner shall
consider current outstanding judgments, current outstanding tax liens,
foreclosures within the past three years, and a pattern of seriously
delinquent accounts within the past three years when considering if a
person is financially responsible. If applicable, provide an explanation as to
the circumstances surrounding any derogatory information and include any
applicable legal documents.

Disclosure Questions: Provide complete details of all events of
proceeding for any "Yes" answer to any of the Disclosure questions for the
company or any Control Person. Upload a copy of any applicable orders or
documents in NMLS.
Commissioners Criminal Conviction Consent Application: Pursuant to
MN Statute 58.125, if applicable, submit the Commissioner's Criminal
Conviction Consent Application form. Include any applicable supporting
documentation and written narratives required by the Application.

Surety Bond. Pursuant to MN Statute 58A.13, subd. 1, each mortgage
loan originator must be covered by a surety bond. In the event that the
MLO is an employee or exclusive agent of a person subject to Chapter 58,
the surety bond of the person subject to this chapter can be used in lieu of
the MLO's surety bond. If applicable use the Surety Bond form attached.
