? Documenting the Public Positions and related Bonding Plans. ? Documenting the Bonding/Surety Agency and Account Numbers. ? Documenting the Bonding/Surety/Insurance Underwriter and Policy Numbers. ? Documenting the Individual's name in each Public Position and their Individual Bonding Plan. ? Documenting the Power of Attorney/Attorney-In-Fact within your organization with the Underwriter. ? Documenting the Complete and Total Coverage Schedule, with all Forms.

We request all fees associated with this request be waived for accessing this public information and a request response be discharged within fifteen (15) business days in accordance with statutory law.

This request is made by Sui Generis. All "citizens", "nationals" and "residents" under your jurisdiction are Sui Generis, by definition, thus denial of Sui Generis for this information is the same as denying your "Citizens"/"Nationals" of this information.

This form is for submitting a Common U.S. FOIA Request for accessing the Bonds/Surety (aka insurance) of a Public Organization. Fill this form out and then file it with any public officials.

This form may be sent by mail, fax, and/or email to the corporation with elected "public officers."

As every incorporated government is a sub-corporation of

WORLD CITIZENS OF UNITED STATES, INC, they are all party to the D.C. Code as this is

where they are all ultimately incorporated (e.g. Receiving an EIN makes it a Federal Incorporation).

HOW TO FILL OUT THIS FORM

To fill out this form, enter your contact information in the left box where the discharged documents are to be received. This may include your name, address, fax number, and/or email address.

The right box is for the Bond/Surety Organization, if it is not with the admission corporation, or if necessary for legal purposes.
