Have several training activities for the staff that include modules and mock patient documents.Recording the information for future staff evaluations and feedback.Implementing a better training for the staff on scanning documents. Lowering the occurrences with scanning for HIPPA violations.The training program that has been identified is scanning process. There is a need for improved training to help minimize the HIPPA violation risks for the company. The company has spent some time and money investing on a new program that has been put into effect. Training the staff to become HIPPA compliant and their attention to details is important when dealing with patient records. Health Insurance Portability and Accountability Act (HIPPA) is federal controlled protection for healthcare of patient information. The patients’ medical information is sensitive. The wrong patient can be told the wrong patient information. This can cause a distress to all parties involved. When the wrong information is entered there can be consequences to the patients’ care by the provider. This new program for the scanning process will help when batching the medical records into the system. This software is designed for scanning of the medical records in the correct place. If an individual who is scanning can stop the batch after every patient chart so the system can detect the break by asking; “would you like to continue with this batch?” Once, the system recognizes the pause it should prompt the individual to notice a different chart was scanned. Utilizing this type of software, there should be minimal occurrences with HIPPA compliance.The resources and budgeting for this new scanning program have been set separately. The management has consulted the providers and they have come up with a scanning tree. This is a go to reference on where exactly the information should be placed once the proper patient has been identified. The scanning tree will be broken down to sub-categories such as outside medical records, clinical notes, labs, radiology, and ER visits. Within these categories the documents will need to be placed for viewing the patients chart. We have identified two programs and decided to start with the one most compatible to the program we already have. This way we can save a on the budget. Software can be pricey for the company and we need one that will be compatible when the upgrades happen in the future. Having access to a technician at our disposal is part of the package. The technician will be able to trouble shoot a problem from a remote location if one should arise. Activities implemented are training modules for all the staff. This way they can be acquainted to the system. Four modules have been set to help the training process. Once the modules are done the staff will do mock test patients in the system. This will be a hands on training. This training will consist of different scenarios for the staff to find problems with and fix. A trainer will be placed at certain locations to help with questions to the program. The training modules will be online for staff to access and do the simulation with the computer. There will be audio scenarios and quizzes for the staff to figure out. The system is designed to educate and not to discourage the staff. The outputs for this training are computer training modules. These modules are done in an eight week program. There are four meeting with a trainer with the specific designed for scanning in records to answer any questions. We will monitor the staff for occurrences after the training and determine if more training is needed. These activities will take place in a three month time period. This way all the staff will have time to do the modules. The activities will be done in three waves with four locations at a time. This way all the attention will focus on a particular site and not the entire twelve clinics at the same time. The staff will be able to give their feedback on the training process once the initial training is completed.The new software will track all the scanned documents by the person who is scanning. This can be a way to track where the initial breakdown is happening and can be addressed. The staff needs to be held accountable for their scanning of the medical documents. The scanning tree will need to be emailed to all the managers to give to staff on a quarterly basis for any changes. The tree will be a references for the staff to place and find medical records on the patients. The short-term outcomes should be successful. The software needs to be user friendly for the staff. In order to lower the occurrences, numerous trainings for HIPPA are now twice a year. The company is keeping all the employees informed and remind of all the possible violations that can occur. Introducing the updated software to the existing one should start lowering the occurrence percentage with HIPPA compliance. Training assessments on the computer to make sure all staff are staying within compliance are documented and reviewed by management on a quarterly basis. The overall long-term outcome will be in the next several month’s tracking data on all the occurrences addressed when scanning medical information since the transition from paper to electronic. It has been noted that thirty percent of the occurrences have been related to scanning documents.
