CPT Manual Usage: Format, symbols, stand alone codes, add on codes, unlisted procedures, guidelines, Category III codes, appendices, PQRI

Modifiers: Level II HCPCS modifiers, modifiers applicable only to procedures; modifiers applicable only to E&M codes

Evaluation and Management Coding: Patient status; place of service and patient type; history, examination and MDM components; counseling, coordination of care, nature of presenting problem, time based coding

Anesthesia and Surgery Coding: Anesthesia coding formula, physical status modifiers, qualifying circumstances, surgical package, global days

Radiology Coding: Plain films, CT, CTA, MRI, MRA, ultrasound, radiation oncology, nuclear medicine. -26 and –TC modifiers

Pathology and Laboratory Coding: Panels, assays, qualitative vs. quantitative, consultations, frozen sections, surgical specimen coding

Medicine Section Coding: Immunizations, hydration, psychiatry, dialysis, ophthalmology, cardiac and nervous system procedures, moderate (conscious) sedation

ICD-9 Coding: Alphabetic index, tabular index, NOS, NEC, “code first”, “code also”, hypertension table, neoplasm table, categories, subcategories, subclassifications, burns, poisonings, V codes, and E codes

Third Party Reimbursement: Medicare, Medicaid, commercial insurance, HMOs, PPOs, RVUs, RBRVS
