TRANSITION FACT SHEET

Students need to apply for services with community agencies as early as possible in order to be sure that appropriate supports are in place upon exiting school. It may require numerous contacts with an agency to stay informed about a student's status on waiting lists for services. Please keep in mind that services and eligibility requirements may vary from year-to-year because of changes governed by state and federal regulations. The following agencies may be contacted at any time for information about services and eligibility services.

EMPLOYMENT SERVICES -- This agency serves individuals with disabilities who eventually expect to function independently in competitive employment. Services may begin during the student’s last year of school.
