10.3 Tip Sheet

Directions for Completion

1. Attendance at a 10.3 class (or training by your unit/department super-user) is required prior to reading the “10.3 Tip Sheet.” Please contact your unit/department super-user if you have not attended a class.

2. The 10.3 Tip Sheet does not replace the content presented in class or by your unit/department super-user.

The tip sheet information provides clarification of some points discussed in the class.

3. Read the entire 10.3 Tip Sheet, which begins on the next page of this document, before proceeding to the posttest.

4. Once completed, exit the tip sheet and complete the posttest which is final step of this education.

← Remember, no attendance record is needed.

← Completion of the posttest will be sent electronically to your EduTracker record once a 100% is achieved.

← Print the Certificate of Completion for your records.

10.3 Tip Sheet

➢ Care Organizer Screen

o Patients can be selected from the drop down box in the patient banner located at the top of the care organizer screen

o All admission information (allergies, home medications, immunizations and admission assessment) are located in Horizon Health Summary

o Patients bracelets should still be scanned in the Care Organizer screen

➢ Horizon Expert Documentation or Chartable Review Screens

o Content has not been changed. The appearance of the screens has changed

o Users now have the ability to toggle between open screens

o In order to close any of the application tabs that are open, right click on the tab and select close. For example: If HHS Allergies is open, right click on the word tab that says HHS Allergies and the word close will appear. Then select close. Using the “x” will result in getting logged out of the Clinicals system.

o Tabs are “hidden” on the right side of the screen. Select the tab on the right to show the hidden tabs, such as radiology, therapy charting, etc.

➢ AdminRx

o When administering medications the end user can see orders which need to be confirmed

o Orders should ALWAYS be confirmed in the Care Organizer screen!!!!!

o Override reasons are done prior to confirming the administration

➢ IV Manage

o Reason must be given when selecting a stock bottle

➢ Horizon Health Summary Allergies

o Every attempt should be made to use a codified allergy

o Codified allergies can be found in the Common Allergies list or by searching

o Bold fields are required to be filled in

o The Verified column has the words “Needs review” in red. This is a feature that cannot be removed. Nurses will confirm the allergies list

o Display can be changed to view All Allergies, Only Active allergies or Inactive allergies

o Column displays can be changed to descending or ascending order by clicking on the column

o If duplicate allergies are documented there is a blue circle with white lines displayed next to it

o The view detail panel will show the history of changes in the documentation of the medication

o Selecting wrong allergen or wrong patient will remove the allergen from the list completely

o Add comment must be selected in order to add a comment to the allergen documentation

o Confirmation of allergies

➢ Can be confirmed individually

➢ The Whole list can be confirmed by highlighting the first allergy, holding the shift key and then selecting the last allergy. All allergies will be highlighted. Then select confirm.

➢ To select multiple items on the allergy list: Highlight the allergies to confirm while holding the control key. Then select confirm

➢ Horizon Health Summary Medications

o Every attempt should be made to use codified medications

o Codified medications can be found in the Common Medications list or by searching

o Bold fields are required to be filled in

o Display can be changed to view All Medications, Only Active medications or Inactive/Discontinued/Held medications

o If duplicate medications are documented there is a blue circle with white lines displayed next to it. They must have something different (dosage etc...) in order to be saved. The system will prompt the user to let them know they are documenting a duplicate medication

o Add comment must be selected in order to add a comment to the medication documentation

o Confirmation of medications

➢ The Whole list can be confirmed by highlighting the first medication, holding the shift key and then selecting the last medication. All medications will be highlighted. Then select confirm.

➢ To select multiple items on the medication list: Highlight the medications to confirm while holding the control key. Then select confirm

o The view detail panel will show the history of changes in the documentation of the medication

o Common dosages and forms of the medication are found in the drop down boxes

o Selecting wrong patient or wrong medication in the Inactive list will remove the medication from the patient completely.

o When done with the medication history, select ready for medication reconciliation

➢ Horizon Health Summary Immunizations

o Every attempt should be made to use codified immunizations

o Codified immunizations can be found in the Common Immunizations list or by searching

o Documentation of immunizations during the hospital stay and prior to admission

o Documentation of immunizations during the hospital stay should be documented in AdminRx. Lot, expiration and manufacturer should be documented in the HHS Immunizations screen for the purpose of running reports.

o When inactivating an immunization by selecting wrong patient, wrong immunization, or not given it will be removed from the patient list

o The view detail panel will show the history of changes in the documentation of the immunization

o Display can be changed to view All Immunizations, Given immunizations or Not given immunizations

➢ Horizon Health Summary Problems List

o Problems will be entered by the Physician and/or extender

o Can be viewed in HHS Problems module

o View can be changed to Active or Inactive Problems

➢ Admission Assessment

o Universal Admission Database for all departments

o Uses branching technology to answer department specific questions

o Full red ellipses (circles) means there has not been documentation in the section

o Half red ellipses means there is partial documentation in the section

o Medication and Allergy history is displayed in the admission assessment. Charting cannot be done in Admission assessment. It must be done in HHS Allergies or HHS Medications

o Immunizations can be viewed as well as if the “…” (edit) box is selected it will take the user to the HHS immunizations charting screen

o By selecting the blue Check mark – This tells other users the section is completed.

o The red null sign should be used in cases when the history is not able to be obtained at the time. Once the history is available it should be documented.

o Calendar buttons are available for known dates throughout HHS

➢ HEO Changes

o Medications are in green.

o Font is different

o Patient banner or F10 button holds functions such as viewing current and completed/discontinued orders, patient allergies and patient demographics

o Physicians can save orders as a draft and activate them when the patient arrives to the floor

➢ Medication Reconciliation

o The new version of Medication reconciliation will allow the physicians and/or extenders to reconcile the patients Home medication list in the web portal.

o Home medication orders can be continued, modified, or discontinued

o If a home medication is discontinued, a reason must be given

o The physicians and/ or extenders can send a message to nursing asking for clarifications from the med rec module

o Physicians and/or extenders can save the med rec as a draft until info is completed.

o The medication orders communicate to HEO

o The physicians and/or extenders can start the discharge med rec at any time during the patients hospital stay and “save as a draft”
