❑ Surgical Services: Premier Contract Launch: Breast Implants, Tissue Expanders and Related Products – (this is in addition to the already existing GNYHA Direct Agreement with Allergan.)

IMPORTANT NOTE: this new Premier agreement is IN ADDITION to the existing GNYHA Direct agreement GNYHA OR-010. Work with your Client Service Executive to identify which program (or hybrid) would work best for you, based on facility product mix.

Premier is pleased to announce its new agreement with Breast Implants, Tissue Expanders and Related Products. The 35.5-month agreement has been awarded to Allergan USA Inc. and is effective February 15, 2011, through January 31, 2014.

❑ Nursing: Price increase on IV glass bottles by Hospira; another product in this category discontinued

Hospira raised the price of three of the IV glass bottles available through Premier agreement PP-IV-045, effective January 1, 2011. The affected IV glass bottles are the 250 milliliter (01614-02), 500 milliliter (01614-03) and 1,000 milliliter (01614-05). The average price increase on these products is 41 percent. Hospira is also discontinuing the 150 milliliter version (0614-01 IV), with no substitute.

On July 9, 2009, Hospira requested Premier remove all of their IV glass bottles from the agreement or increase the price of the products but the request was rejected. In fall 2010, Premier reviewed the market information regarding the rising cost of hospital-grade glass and determined a price increase would be justified. Premier set an effective date of January 1, 2011.

❑ High Level Disinfectants launch effective May 1, 2011

High-level disinfectants are chemicals that are capable of killing bacteria, viruses and bacterial spores when used in sufficient concentration under suitable conditions and are primarily used to disinfect semi critical items. Also included in this category are Intermediate-level disinfectant wipes which are used to disinfect patient care areas and equipment in between patient use.

Premier’s Nursing Committee is pleased to announce new agreements in High Level Disinfectants have been awarded to incumbents Metrex Research, LLC and Johnson & Johnson Health Care Systems Inc. (J&J); and new suppliers Certol International, Clorox Sales Company, and Professional Disposables

❑ Sharps Disposal Containers effective May 1, 2011

Sharps Disposal Containers:

This category includes the canisters designed for the safe containment of sharps medical waste and the related accessories such as wall mountings and brackets. This category does not cover reusable containers or services for the removal or processing containers as those are covered under other categories.

❑ Annual price increase on IV therapy products by Baxter Healthcare Corporation

Baxter Healthcare Corporation notified Premier of its right per the terms and conditions to implement an annual inflation price increase for the IV therapy agreements (PP-IV-013, PP-IV-023, PP-IV-033, PP-IV-043), effective February 1, 2011. Tiers 1 through 5 will have a 4 percent increase and pricing will increase by 3 percent in Tiers 6-8. The total in aggregate impact of this price increase to the membership is 3.3 percent.

❑ Baxter issues urgent device correction on AUTOMIX compounder

In a letter dated January 27, 2011, Baxter Healthcare Corporation voluntarily notified its customers of a potential keypad issue with the AUTOMIX compounder, available through Premier agreement PP-IV-043. Baxter is investigating the cause of the issue described in the letter and will contact customers as a follow-up within 90 days. All personnel who use the compounder should verify data accuracy each time the keypad is used and notify Baxter’s global technical services at 1.800.626.2667 should they experience an incorrect admixture display.

❑ Johnson & Johnson – Gas Plasma Sterilization

CONTRACT EXTENSION: The Gas Plasma Sterilization contract with Johnson & Johnson (PP-OR-526) has been extended until February 29, 2012. This contract was originally set to expire February 28, 2011. The extension includes a weighted average price increase to the consumables on this contract of 2.6 percent. The price on the capital equipment for this contract will not change. The overall weighted average price increase (capital and consumables) is 1.6 percent.

Category history: On March 1, 2009, a new portfolio of agreements began for Gas Plasma Sterilization with Steris (PP-OR-527) and Johnson & Johnson (PP-OR-526). The Johnson & Johnson agreement was a 12-month agreement with an option of two one-year extensions. The contract was extended once from February 28, 2010, to February 28, 2011. As the agreement was set to expire February 28, 2011, the decision to extend the agreement for an additional one-year term, through February 29, 2012, has been mutually agreed upon between Premier and Johnson & Johnson.

❑ Johnson & Johnson – Automated Endoscopic Reprocessors

CONTRACT EXTENSION: The Automated Endoscopic Reprocessors contract with Johnson & Johnson (PP-OR-537) has been extended until February 29, 2012. This contract was originally set to expire April 30, 2011. The extension includes a 2.2 percent price increase to two consumables on the contract (item #s 20398, 22591). The price on all other products will not change. The overall weighted average price increase to the contract (capital and consumables) is .7 percent.

Category history: On March 1, 2009, a new portfolio of agreements began for Automated Endoscopic Reprocessors with Minntech (PP-OR-517) and Steris (PP-OR-516). The Johnson & Johnson agreement for this category began May 1, 2009, and was a 12-month agreement with an option of two one-year extensions. The agreement was extended once from April 30, 2010, to April 30, 2011. The decision to extend the agreement for an additional 10-month term, through February 29, 2012, has been mutually agreed upon between Premier and Johnson & Johnson. The 10-month extension will make the Johnson & Johnson agreement co-terminous with the other agreements in the category.

❑ Nursing RECALL – announcement that Smith Medical ASD Inc. (PP-IV-018) is conducting an urgent voluntary correction of a limited number of CADD-Solis ambulatory infusion pumps.

This corrective action is due to increasing reports of the cassette lock mechanism on certain CADD-Solis pumps becoming stuck in the locked position, making it nearly impossible to remove the medication cassette reservoirs (or administration set). This urgent medical device correction only applies to the pump serial numbers listed in the attachment. While not all pumps on the list have experienced this issue, Smiths Medical is proactively providing this correction to all pumps on the list.

❑ Imaging – announcement that Rays (fks NightRays) PP-IM-134 is offering discount off professional fees for teleradiology services. Promotion ends March 1, 2011.

❑ Nursing Product Alert – announcement that Medical Action Industries Inc. (PP-NS-622) has issued a voluntary drug recall by the Triad Group for all lots of alcohol prep pads, alcohol swabs and alcohol swab sticks, both sterile and non-sterile. The following Triad Group items are used in Medical Action Industries kits or trays: 11SA01, 11SA03, and PL3536. Medical Action has implemented a field correction process and is working with distributors to ensure that all affected products are identified, removed and returned.

❑ Continuum of Care – announcement of new agreements for EMS Products. These agreements provide a comprehensive range of durable emergency medical service equipment to first responders and emergency personnel.

❑ Materials Management – announcement that J & J has changed their business model for all Ethicon & Endo products. They have changed their payment terms with Distributors for all J & J Medical Division products. Distributors most likely will be passing this increase on their customers at a 1.5% - 2% price increase.

• J&J has changed their business model by now offering suture and endo direct and without a cost-plus mark up.  Standard delivery for these products will be 2nd day delivery.

• J&J will now offer the Harmonic Scalpel through distribution with same fees as Ethicon & Endo.  Unfortunately, Harmonic Scalpel is not on a Premier contract.

• J&J will now offer a growth incentive for Ethicon for both acute and non-acute distributors.

• J&J indicates that these changes offer choice of channel for customers, as well as it offering a less expensive option for members to purchase direct vs distribution

❑ RECALL Surgical Services – announcement that Davol Inc. (PP-OR-688) has issued a voluntary recall of the XenMatrix Surgical Graft.

The voluntary recall has been initiated because testing cannot ensure that all units of the XenMatrix Surgical Graft are within FDA guidelines for endotoxin requirements. No patient injuries or complications have been reported that have been linked to elevated endotoxin levels.

❑ RECALL Surgical Services – announcement that ConMed Electrosurgery (PP-OR-665) has issued a recall on System 2450 Electrosurgical Generator.

ConMed Electrosurgery has determined a component within the System 2450 may cease to perform properly, which may, given the completion of a number of events, result in an electrical burn to the ESU operator or patient.

❑ Nursing – announcement that Baxter Healthcare Corporation will discontinuation of the Ipump Pain Management System, which had been available through Premier agreement PP-IV-013. Baxter ceased the sale of Ipump, Product Code 2L3107, on December 31, 2010. Baxter will continue to repair and service parts for the Ipump for another year (through December 31, 2011), as long as components remain available through suppliers. A limited pool of loaner pumps will be available. Disposable administration sets and accessories used with the Ipump will continue to be available for sale.

❑ Nursing Price Increase – announcement that effective February 1, 2011 Baxa Corporation (PP-IV-041) will implement a 3.5% line item price increase on the non-TPN product line.

❑ Nursing Price Increase – announcement that effective February 1, 2011 Hospira Worldwide will implement a price increase. The price increase pertains to IV fluids (PP-IV-035), sets and tubing (PP-IV-025) and syringe pumps (PP-IV-015).

❑ Laboratories – announcement LifeScan (PP-LA-260) to exit hospital market for blood glucose in two years.

❑ Laboratories – announcement that Cepheid (PP-LA-226) is offering a promotion for PCR Molecular Diagnostic Instruments and Assays. Contact Ken Gordon, director, corporate accounts at 609.284.5876.

❑ Laboratories – announcement of a Breakthroughs Award for Open Channel Reagnets: HIV Ag/Ab Assay Technology.

The agreement with Abbott Laboratories Inc. will be effective January 1, 2011, through March 31, 2013, making all contracts in the Open Channel Reagents category co-terminus. This new assay was determined by the Premier member Clinical Laboratory Services Committee to be unique in the marketplace. The assay is a combination test that detects HIV-1 antibodies, HIV-2 antibodies, and

HIV-1 p24 antigen, allowing complete sensitivity across multiple strains and earlier detection of HIV infection.

❑ Laboratories RECALL – announcement that Abbott Point of Care (PP-LA-236) has initiated a recall on the specific lots of Cardiac Marker Control Levels 1, 2, 3 and Cardiac Marker Control Calibration Verification Sets. Lots M102081, M102082, M102083, M10208.

Abbott Point of Care has determined that for the above listed Cardiac Marker Controls and Calibration Verification fluid lots, results may be generated above the upper value assignment range for some vials. Internal studies have determined that the cause of the issue is CO2 emitted by dry ice, which penetrates into the vials (head space) during the shipment process that is the cause of the high results. Venting of the vials prior to thawing prevents the occurrence of high results.

Abbott Point of Care recommends that prior to thawing the vials from the lots listed above:

• Remove cap from vial.

• Squeeze vial 10 times, IMMEDIATELY when the vials are taken out of the freezer.

• The vial must be squeezed 10 times before the thawing process.

• Replace cap on vial.

• Follow remaining steps for use as outlined in section 14-12 of the iSTAT 1 System Manual (ART: 714376-00K).

❑ Women and Children – announcement of a new agreement for BreastPump Equipment and Accessories.

❑ Women and Children – announcement that SmileMakers has cancelled/terminated their Premier agreement, effective January 31, 2011.

❑ Surgical Services RECALL – announcement that ConMed Linvatec (PP-OR-639) has issued a recall of a Micro Sagittal Blade, catalog # 5032-138. These products may have a breach in the seal between the Tyvek lid and tray, which could potentially comprise the sterility of the device.

❑ Surgical Services RECALL – announcement that Integra Surgical (PP-OR-611) has issued a voluntary recall of specific part numbers of the Jarit Secure-Cut Reposable Scissors.

Effective immediately, members should cease using all products with the affected associated product codes. The impacted product codes are 625-050 and 625-051. This recall has been initiated as Integra Surgical has received reports instances where the outer shaft sheath has advanced forward during reprocessing and thus occluded access to the threads where the disposable scissor tips would attach. Tip part numbers 625-060, 625-061, 625-062, 625-063 are rendered obsolete and discontinued due to the handle recall.

This recall is being conducted with the knowledge of the U.S. Food and Drug Administration.

❑ Surgical Services – announcement that Prestige Ameritech as added it new ProGear N95 Respirator to agreement PP-OR-705. This N95 respirator has the highest fluid-resistance rating available.

❑ Surgical Services – reminder that Angiodynamics (PP-OR-728) has been added to the Implantable Infusion Ports agreement, effective December 1, 2010. PMDF or price activation is required.

❑ Nursing – announcement of new agreements for Negative Pressure Wound Therapy. Clinical Field Guide is available upon request. Call your Client Services Executive to request.

❑ Nursing – announcement that Baxter Healthcare Corporation has notified Premier that they will be implementing an annual inflation price increase. Increases involve IV therapy agreements (PP-IV-013, PP-IV-023, PP-IV-033, PP-IV-043), effective February 1, 2011. Tiers 1 through 5 will have a 4 percent increase and pricing will increase by 3 percent in Tiers 6-8. The total in aggregate impact of this price increase to the membership is 3.3 percent.

❑ Nursing – announcement that KCI V.A.C. VIA therapy system has be added to the Negative Pressure Wound Therapy agreement PP-NS-438, effective November 15, 2010. This seven-day disposable system offers portable therapy and allows for earlier patient discharge by reducing or eliminating the need for insurance authorizations.

As a special introduction, KCI will provide a rebate for each VPC card submitted the day after the patient placement of $50 for fax or phone submissions and $100 for KCI Express submissions. The offer ends December 31, 2011. Please reference Exhibits P-1 and P-2 in Supply Chain Advisor® for details.

❑ Nursing – announcement that Edwards Lifesciences has announced a new kitting and distribution agreement with Centurion Medical Products. Products are available through Premier agreements PP-NS-508 Invasive Hemodynamic Monitoring Products and Accessories and PP-NS-526 Central Venous Access Products. This transition of products to Centurion affects the Access product

portfolio, which includes the following Edwards products: Multi-Med™, Vantex™, Introflex™, and AVA Advanced Venous Access. Order placement is now with Centurion. Please call 800.248.4058 for all Access orders as well as fax, GHX and all other EDI orders. Distribution customers may continue to contact their distributors as usual.

❑ Surgical Services – announcement that Premier has been notified by Integra LifeSciences (PP-OR-478) of their intention to implement an annual price increase of 3% effective January 1, 2011.

❑ Recall Surgical Services – announcement that Covidien (PP-OR-664), formerly Valleylab) has issued a voluntary recall of specific lot numbers of their Ligasure Blunt tip Laparoscopic Sealer/Divider.

Effective immediately, members should cease using all products with the affected lot numbers and associated product codes. The impacted product codes 188856, 190048, 190107,190307 and 193333. This recall has been initiated as Covidien has received reports that during surgical procedures, the device may energize when clamping without pushing the activation button.

This recall is being conducted with the knowledge of the U.S. Food and Drug Administration. Customers who have been shipped product from any of the affected lots have been notified of this recall.

Suggested actions:

• Stop using affected product immediately.

• Quarantine and remove all stock on hand.

• Arrange for product returns.

• Complete the Domestic Response Form.

❑ Surgical Services – announcement that Davol Inc. (PP-OR-673) will increase Tier 1 prices for their Surgical Mesh effective January 1, 2011. The increase will be 3% for all but 9 products.

❑ Surgical Services – announcement of new agreements for Arthroscopy Fluid Waste Management has been awarded, effective March 1, 2011 through February 28, 2014.

These products included in this category include suction devices, accessories and absorbent floor pads. These devices are used to remove fluid from the operating room floor during the surgical procedure. These products keep the area dry, prevent equipment damage, keep the area free of contamination, and help prevent staff falls on wet floors. The suction devices tubing is purchased separately and attaches to the suction canister.

❑ Surgical Services – announcement that Davol Inc. has added Ventalight ST surgical mesh to agreement PP-OR-673. This offering for those surgeons who clinically support the need for lighter weight mesh (below 52 grams per metered square) for the repair of ventral hernias.

❑ Surgical Services – announcement that B. Braun has been added to the Regional Anesthesia Trays and Related Products agreement effective November 22, 2010 along with Arrow International, BD and Company and Smith Medical. All agreements expire January 31, 2013.

❑ Nursing – announcement that the Kimberly-Clark KimVent 24-Hour Oral Care q2 and q4 kits with CHG have now been added to Premier agreements PP-NS-604 effective December 1, 2010. The Kimberly-Clark KimVent 24-Hour Oral Care q2 and q4 kits with CHG are comprehensive, flexible, easy-to-use systems that contain everything needed to clean, debride, suction and moisturize

❑ Nursing – announcement that the Compression Therapy Concepts (CTC) VasoPress Supreme Mini pump model VP500DM has been added to agreement PP-NS-576, effective October 1, 2010. This product allows for portability and versatility of transport in a smaller battery pump.

❑ RECALL Nursing – announcement of a SIGMA Spectrum Infusion Pump Model 35700 Recall.

The SIGMA Spectrum Infusion Pump is intended for the delivery of fluids, solutions, drugs, agents, nutritionals, electrolytes, blood and blood products via parenteral, enteral, intravenous, intra-arterial, subcutaneous, epidural, or irrigation routes of administration.  

These units may fail suddenly causing inaccurate flow conditions during use, ranging from back flow to over-infusion, including free flow. The pump does not issue an alarm when this occurs. These conditions could result in serious injury or death. For further details, please visit FDA's website.

❑ Surgical Services – announcement that Ecolab Inc. (PP-OR-534) will discontinue ENDURE 450 Waterless Hand Scrub. This will immediately affect the following product codes:

• Item # 61094520 : Endure 450 6-1000mL (bag)

❑ Surgical Services – announcement that Covidien Price Protection on previous surgical energy agreement ends November 30, 2010. Effective December 1, 2010, pricing at Tier 1 will change and will be subject to future price increases.

Pricing is firm for the first two years, with a 2 percent increase in the third year. The agreement allows for a 5 percent increase for Tier 1 and a 3 percent increase for Tiers 2-4 in the fourth year. Today, some products at Tier 1 pricing will be lower than the higher tiers. There are no changes to Tier 2, Tier 3 and Tier 4 prices in the new surgical energy agreement, PP-OR-664.

❑ Surgical Services – announcement that Surgical and Specialty Blades agreements are transitioning away from BD.

In a letter dated June 8, 2010, Becton, Dickinson and Company (BD) announced its intent to sell certain assets of the BD Medical segment, including its surgical and specialty blades platforms. The surgical and specialty blades business has now been sold to Roundtable Healthcare Partners. Two of Roundtable’s subsidiaries, Aspen Surgical Products and Beaver-Visitec International, will assume contracts PP-OR-653 (surgical blades) and PP-OR-468 (ophthalmology) in full with no changes in terms. The contract assignment to Aspen Surgical for PP-OR-653 has been completed and the assignment to Beaver-Visitec for PP-OR-468 will be completed. Members should continue to operate as normal in regards to both agreements.

What to expect during the transition:

• BD will continue servicing both agreements until the other entities assume full operation.

• Distribution orders should remain seamless.

• This primarily pertains to direct orders.

❑ Nursing – announcement that Teleflex has added the ArrowEVOLUTION PICC to Premier agreement PP-NS-527. This new peripherally inserted catheter (PICC) is designed to provide an initial burst of chlorhexidine after insertion to protect the subcutaneous tract.

❑ Nursing – announcement of new agreements for Adhesive Skins Closures effective February 1, 2011. Products in this category include adhesive tape-like

strips for use in supporting the closure of a skin wound/laceration such as Steri-StripTM and Butterfly strips. It also includes Tincture of Benzoin, which is often applied to skin before applying tape or other adhesive bandages

❑ Nursing – announcement of new agreements for Tape products and Cohesive Bandages. Tape products are strips of a variety of strong woven fabric or other materials used to bind and hold bandages in place. Cohesive bandages are self-
