ACCEPTABLE USE OF INFORMATION TECHNOLOGY SERVICES AUTHORIZATION

The Ewing Public Schools District is committed to celebrating our students’ successes and accomplishments, highlighting the education process and providing information by filming and/or photographing students in only the most positive view.

This includes articles and pictures of students appearing in print (e.g., newspapers, newsletters and yearbooks, etc.) and on district school websites. As you are aware, there are potential dangers associated with the posting of personally identifiable information on a website since global access to the Internet does not allow us to control who may access such information. These dangers have always existed; however, the District does want to celebrate our students’ accomplishments.

The Family Educational Rights and Privacy Act (FERPA), a Federal law*, requires that The Ewing Public Schools District, with certain exceptions, obtain written consent of the parent/guardian prior to the disclosure of personally identifiable information from your child's education records. Pursuant to law, we will not release any personally identifiable information without prior written consent from parents/guardians.

Personally identifiable information includes student names, photos or images. (Please note the District does not publish photos with residential addresses, e-mail addresses, phone numbers or the locations and times of class trips on the school or district web sites.) Directory information, information that is generally not considered harmful or an invasion of privacy if released, can also be disclosed to outside organizations without a parent/guardian’s prior written consent. Outside organizations include, but are not limited to, companies that manufacture class rings or publish yearbooks or media publications. In addition, two federal laws* require local educational agencies (LEAs) receiving assistance under the Elementary and Secondary Education Act of 1965 (ESEA) to provide military recruiters, upon request, with three directory information categories (names, addresses and telephone listings) unless parents have advised the LEA that they do not want their student's information disclosed without prior written consent.

The Ewing Public Schools District has designated directory information includes, but may not be limited to, the following:

- Student's name - Date and place of birth - Participation in officially recognized activities and sports

- Address - Major field of study - Weight and height of members of athletic teams

- Telephone listing - Dates of attendance - Degrees, honors, and awards received

- Electronic mail address - Grade Level - Most recent educational agency or institution attended

- Photograph - Playbills - Annual Yearbooks

- Honor roll or other awards - Sports activity information - Videos of awards, recognition or promotion ceremonies, special events and concerts

*Section 9528 of the ESEA (20 U.S.C. 7908), as amended by the No Child Left Behind Act of 2001 (P.L. 107-110), the Education Bill, 10 U.S.C. 503, as amended by Section 544, the National Defense Authorization Act for Fiscal Year 2002 (P.L. 107-107), the legislation that provides funding for the Nations' armed forces

The Right to Opt Out/Change Your Mind

The District may disclose appropriately designated ‘directory information’ without written consent, unless you have advised to the contrary in accordance with District procedures. Once you have given signed permission for the above options, you may change your mind. As the parent or guardian wishing to change this agreement, you may do so at any time by sending a letter to the principal of your child's school. The change will take effect upon receipt by the school.

Please complete the Web-Media-Directory Information Release Authorization/Acceptable Use Acknowledgement form and return it to the school office by September 15th of the current school year or within 15 days of entering The Ewing Public Schools District.

Please be sure you have read the explanation of Web-Media-Directory Information Release on the reverse side, as well as Policy/Regulation 2361 - Acceptable Use of Computer Networks/Computers and Resources (available on the website). This acknowledgement is required when students enter the District and again when entering the 6th and 9th grades.

If you do not want your child’s directory information released (review explanation of Directory Information listed in this document), parent/guardian initial below.

NO, I DO NOT GRANT PERMISSION for the release of web-media-directory information. I do not grant permission for a photo/image that includes the student to be published in any media outlet (both print and web) or on the district's public Internet site. I am aware students may appear in large group photos with no personal identifier such as, but not limited to concerts, award, recognition or promotion ceremonies, and programs aired on the district’s educational channel.

Section 2: ACCEPTABLE USE POLICY/REGULATION

Sign and return this form in order to use district information technology services.

To use the district information technology services, including Internet access, provided by The Ewing Public Schools District, we have read P/R 2361 Acceptable Use of Computer Networks/Computers and Resources. We acknowledge the potential for students to access inappropriate information, and that while the district restricts connection to inappropriate sites, students ultimately are responsible for the information they access.
