Position Summary

The New Foundations Coordinator is responsible for coordinating housing services, collaborating with area providers, and ensuring continuity of care for New Foundations. The service is provided in an effort to provide stabilized housing, decrease voluntary hospitalizations, and ensure follow- up care.

The New Foundations Coordinator must demonstrate strong skills in establishing priorities, assigning and reviewing duties and tasks to staff, and resolving issues in the New Foundations Program. In order to manage multiple participants and staff, the Coordinator must demonstrate the ability to multi-task and delegate to achieve desired end results. The Coordinator should also demonstrate good communication skills, skillfully identify issues, and provide support to staff.

Responsibilities:

1. Complete all documentation required for New Foundations according to the guidelines. Documentation is to be completed appropriately, legibly, and in a timely manner.

2. Provide transportation for participants to/ from New Foundations when necessary and as assigned by the Director of Mental Health Programs.

3. Review all New Foundations admissions, ensuring all criteria is met and orientation packets are complete.

4. Be available via phone to support staff in the absence of the Director of Mental Health Programs.

5. Work cooperatively with Behavioral Health & Developmental Services, Westmoreland Casemanagement and Supports, other mental health providers and other human service providers for housing planning.

6. Coordinate all admissions to and discharges from New Foundations, including linkages with providers in the community. Consult with the Director of Mental Health Programs as needed, ensuring all admissions to New Foundations are appropriate.

7. Schedule and conduct all discharge planning meetings for New Foundations.

8. Attend all mandatory trainings, conferences, and staff meetings as scheduled by the Director of Mental Health Programs.

9. Ensure New Foundations is operated in accordance with CSP principles.

10. Ensure participant confidentiality, right to appeal and individual civil rights are maintained in compliance with HIPAA standards established by Westmoreland Community Action.

11. Maintain compliance with HIPAA regulations, in regard to accessing Protected Health Information, according to the Table of Access contained in the HIPAA Policy and Procedure Manual. Employee’s access is on a “need to know” basis.

12. Conduct bi-monthly staff meetings in the absence of the Director of Mental Health Programs.

13. Assist with the day- to- day operations of the facility. This is to include: fiscal, maintenance, purchase orders, and fire drills.

14. Perform any and all other duties as assigned by the Director of Mental Health Programs or VP of Behavioral Health Services.

15. Work independently and maintain accurate, detailed Program documentation.

16. Represent the Program in the community and conduct public education on the Program as assigned.

17. Oversee the scheduling of staff, ensuring 24/7 coverage of the Program in the absence of the Director of Mental Health Programs.

18. Provide feedback and support to staff, increasing their ability and skill set.

19. Educate the staff on Westmoreland County community resources aimed at assisting persons with mental illness, as well as, other special populations.

20. Assess the quality of documentation provided by support staff, facilitating staff development to improve documentation skills.

21. Identify staff training needs and develop ongoing training for New Foundations staff.

24. Collaborate with local providers, ensuring continuity of care.

Education, Skills and/or Experience

1. Bachelor’s degree with major course work in either sociology, social work, psychology, gerontology, criminal justice, counseling, education or a related field or

2. One (1) year previous casework experience – preferably in the mental health field.

3. Direct experience in crisis intervention with the ability to assess crisis situations, respond by providing appropriate counseling, assist in resolving crisis situations and arranging for efficient referral and follow-up services.

4. Knowledge of counseling skills.

5. Knowledge of the components of family therapy helpful.

6. Knowledge of dialectic and cognitive behavioral therapy helpful.

7. Ability to provide accurate written documentation of all crisis contacts.

8. Ability to work with all age groups in resolving crisis situations.

9. Ability to work without direct supervision and maintain accurate, detailed case documentation.

10. Ability and willingness to work various shifts, and perform the duties specific to the Crisis Programs.

11. Must carry 100,000/300,000 bodily liability car insurance.

12. Must complete a post job offer physical examination form in compliance with Chapter 5310.21 (e) of the Commonwealth of Pennsylvania Mental Health Manual. This form includes a screening for tuberculosis, is signed by a licensed physician and certifies the employee is free from any communicable diseases.

Conceptualization - Able to understand and relate to theories behind several related concepts.

Memory - Able to remember multiple verbal and written task-

assignments given at beginning of a period extending over long period of time.
