Additional Factors Added to NYC Standardized Vulnerability Assessment

The Supportive Housing Applications (HRA 2010e) now includes new questions to assess for additional vulnerability factors for specific populations. Based on community feedback from supportive housing providers, government stakeholders, persons with lived experience, and homeless services membership organizations, it was determined that these new factors would more effectively capture the vulnerability of the identified populations. The U.S. Department of Housing and Urban Development's (HUD) allows communities to determine additional factors for specific homeless populations such as young adults, survivors of intimate partner violence, families with children, and unsheltered individuals.

No new additional documentation will be required however the application should be completed in its entirety ensuring that the responses accurately reflect the applicant's history and experience.

The Vacancy Update is published every two weeks by the Residential Placement Management System (RPMS) of the CUCS Housing Resource Center and is supported by the NYC Department of Health and Mental Hygiene. NYC Standardized Vulnerability Assessment (SVA) Additional Factors

Effective May 17, 2019, the Supportive Housing Application (HRA 2010e) will include new questions to assess for additional vulnerability factors for specific populations. Based on community feedback from supportive housing providers, government stakeholders, persons with lived experience, and homeless services membership organizations, it was determined that these new factors would more effectively capture the vulnerability of the identified populations. The U.S. Department of Housing and Urban Development's (HUD) allows communities to determine additional factors for specific homeless populations such as young adults, survivors of intimate partner violence, families with children, and unsheltered individuals. For more information you may visit the HUD exchange to review notice 24 CFR 578.7(a)(8) and subsequent notices requiring a centralized and/or coordinated assessment system.

Where can I get more information on how the additional NYC SVA factors are determined?

Referring agencies completing supportive housing applications are strongly encouraged to complete the Understanding the NYC Standardized Vulnerability Assessment training. To learn more about the factors informing the SVA review the Standardized Vulnerability Assessment Criteria Fact Sheet. The updated fact sheet and training can be found in the What's New section of PACT System.

Will additional documentation be required?

No, however the application should be completed in its entirety ensuring that the responses accurately reflect the applicant's history and experience. Where applicable, the psychosocial assessment and/or psychiatric evaluation should be consistent with the information contained in the HRA 2010e. Additional information and/or documentation may be requested for further assessment.

How does the NYC SVA impact an applicant's referral to supportive housing?

Referrals to supportive housing programs receiving HUD funding as a part of the NYC Coalition on the Continuum of Care (CCoC) will be conducted in accordance to the CCoC's written standards. To review the CCoC's written please visit their website at .

Additionally, referrals to NYC 15/15 supportive housing programs will be prioritized utilizing the SVA.

New 2010e Housing and Homeless History Requirements

The New York City Supportive Housing Task Force made recommendations for NYC's 15/15 Supportive Housing Initiative to align the definition of chronic homelessness with the U.S. Department of Housing and Urban Development (HUD). This chronic homeless definition is applicable to the NYC 15/15 Singles, Adult Family, and Families with Children populations. Also, all supportive housing programs that receive CoC funding are required to ensure applicants meet HUD's definition of chronic homelessness. The homeless criteria for all other supportive housing agreements or types will not change.

Effective December 7, 2018, the Supportive Housing Application (HRA 2010e) will include a new requirement for the referring worker to record the complete housing history of an applicant for the past four years. A new function in the application will assist the user to complete this section by displaying emergency housing information from HRA and DHS system databases (if applicable), as well as housing history reported in prior HRA 2010e applications. Additionally, a New York City Housing and Homeless History Report will be included with all approved applications. This report provides a detailed history of the four year housing information that was verified from the application and emergency housing system databases.

Cisco Jabber QuickGuide Cisco Jabber Guest (PC)

External users may have to contact providers through Cisco devices (ie. Cisco DX-80). To do this, the application Cisco Jabber Guest must be installed on your PC. This guide covers steps on how to install Cisco Jabber,

and how to place calls with providers using your PC and a webcam.

Conferencing With a Provider

1. You will receive a link via email from a H+H provider. Keep this link, as this is what will be used to place the call. Click on the link.

2. A prompt to install the Cisco Jabber Guest extension to the computer will appear.

3. Add Cisco Jabber Guest to your browser.

4. Re-open the link sent from the provider. Install Cisco Jabber Guest Add-On.

5. Application should load with contact's number at the top. If attached, webcam will also load. Make sure to Allow access to microphone and camera when prompted by your PC.

6. Click Call. Device will attempt to connect to the provider.

7. Click End Call to disconnect.
