Applications are invited for two posts of Consultant Physician with an interest in Gastroenterology & General Medicine.

Posts are available on Gartnavel General / Western Infirmary and Royal Alexandra Hospital.

All posts take part in acute medical receiving and provide care for inpatients, outpatients and daycases. The endoscopy departments provide the full range of diagnostic and therapeutic endoscopy. The Western Infirmary has an emergency endoscopy rota. Sub-speciality services within the service include hepatology, inflammatory bowel disease, clinical nutrition and upper gastrointestinal disease and the successful candidates will have the opportunity to develop a relevant sub specialty interest. Clinical Nurse Specialists provide support in Hepatology, IBD, Viral Hepatitis and upper and lower endoscopy.

The specialty has a strong academic background and close links with the University of Glasgow, as well as close clinical and educational links between units across the city.

The posts comes at a time of major changes in the way clinical services are delivered in Greater Glasgow and Clyde, and will be integral to the planning and development of Gastroenterology services in the city in the coming years. In 2015 the new Southern General hospital will open. This is a purpose built hospital into which services from the Victoria Infirmary, Southern General Hospital and Western Infirmary will move.

Applicants must have full registration with the General Medical Council and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher Specialist Training leading to a CCT in Gastroenterology and General (Internal) Medicine or be within six months of confirmed entry from the date of interview. Non UK applicants must demonstrate equivalent training.

Acute Services Division, NHS Greater Glasgow and Clyde

The Acute Division of NHS Greater Glasgow and Clyde is the largest group of adult acute hospitals in Scotland – offering many opportunities to ensure job satisfaction and career development.

Department of Gastroenterology

The Department of Gastroenterology is based at Gartnavel General Hospital, and is part of the Acute Services Division, NHS Greater Glasgow and Clyde and resides within the Emergency care and medical services division.

The department provides Gastroenterology and Hepatology services to both Gartnavel General Hospital and the Western Infirmary, serving North West Glasgow. The Department is based on Ward 8C (24 beds) with significant services provided through the recently redeveloped Medical Day Unit, the Gartnavel Hepatitis Centre, and the Outpatient Department. Gartnavel has a 3 room endoscopy department offering diagnostic and therapeutic upper and lower gastrointestinal endoscopy and ERCP, in addition a 24hour emergency endoscopy rota is provided for the Western Infirmary. Currently the department consists of 5 consultants with interests in Inflammatory Bowel Disease, Dyspepsia, Hepatology and Clinical Nutrition. Additionally the department is supported by a Hepatology Nurse Specialist, IBD Nurse Specialist, two Hepatitis Nurse Specialists, a dedicated Research Nurse and upper and lower Nurse Endoscopists.

The successful candidate will be encouraged to develop a subspecialist interest within the fields of either Luminal Gastroenterology or Hepatology.

The Gartnavel site is also home to the Beatson West of Scotland Cancer Centre, the West of Scotland Cystic Fibrosis Unit and Scottish Pulmonary Vascular Unit, all of which require significant gastroenterology input, and it is hoped that this post can develop links further with these departments. There is also a well established interventional radiology department which works closely with the Department of Gastroenterology.

There is a large cohort of hepatitis C infected patients (approximately 1700 individuals) attending a dedicated hepatitis C service. In collaboration with the Department of Infectious Diseases and the Greater Glasgow & Clyde Hepatitis C Managed Care Network, major service developments for the testing, treatment and care of HCV infected individuals are underway. These include the newly developed Gartnavel Hepatitis Centre, and expanding community and prison based HCV services throughout the Greater Glasgow & Clyde Health Board Area.

The Department has close links with the University of Glasgow, Professor KEL McColl leads the University Department of Gastroenterology, which has an internationally renowned reputation in the fields of dyspepsia, gastro-oesophageal reflux and the pathogenesis of upper gastrointestinal cancer. Additionally, the Department has a long track record of Hepatology research, particularly in the field of viral hepatitis, and is currently involved in a number of studies and clinical trials. It is hoped that the successful applicant would eventually be able to contribute to these areas of interest.

The Gastroenterology Unit at Gartnavel already has close links with other units across the city, and it is proposed that when the New Southern General Hospital opens in 2015, the Gastroenterology units at Gartnavel, the Southern General Hospital and Victoria Infirmary will merge at the new Hospital. This will allow provision of a comprehensive Gastrointestinal Bleeding service, and more focused Gastroenterology input into acute receiving.

General Internal Medicine (GIM)

The department covers all aspects of investigation and treatment of acute and non-acute medical patients with corresponding outpatient clinics and wards in the Western Infirmary and Gartnavel General Hospital. Acute Medical Receiving is undertaken on a rotational basis between teams based at both hospitals. However, all acute medical patients are initially admitted to the 28 bed Medical Assessment Unit (MAU) at the Western Infirmary, and are later triaged to general medical or other specialist units wither within the Western Infirmary or Gartnavel General Hospital. The receiving physician of the day undertakes an evening ward round on the MAU and a post-receiving ward round the following day.

4. Duties of the post:

The duties of the post holder will be provision with Consultant colleagues of all aspects of the service in Gastroenterology and Hepatology, including participation in the haematemesis on-call rota. This will include clinical care of inpatients and out patients, audit, teaching, research, and the provision of advice by telephone and formal consultation to other hospital departments, and through administrative and committee work.

The post holder will contribute to the care of General Medical patients. These responsibilities will include contribution to the acute medical receiving rota and an approximately 1 in 25 basis.

a) Teaching, audit, and research:

The post holder will be expected to contribute to the clinical audit programme and clinical governance programme within the Department. With time the post holder will also be expected to contribute to the undergraduate and post graduate teaching programme.

The post holder will be expected to develop their research interests to complement those currently undertaken in the Department of Gastroenterology.

b) Administration and management:

The post holder will undertake the administrative duties associated with the care of his/her patients and in the running of the department.

The post holder will assist with junior doctor induction, educational activity, and rotas within their SPA allocation.

c) Qualifications:

The post holder will hold the MRCP(UK) or equivalent, and will be within 6 months of completion of higher professional training in Gastroenterology and General Medicine at the time of interview.

Royal Alexandra Hospital

This post of Consultant Physician with an interest in Gastroenterology will be based at the Royal Alexandra Hospital (RAH), Paisley, five miles from the bustling West end of Glasgow. This post offers a real opportunity for the successful candidate to be involved in the development of a modern consultant led service within Gastroenterology.

1. General Information

The Royal Alexandra Hospital (RAH) is situated in Paisley and provides acute health care services to Renfrew District (pop 205,000). The Hospital is a 968 bed modern District General Hospital (DGH) which opened in 1986 and is one of the largest and busiest non-teaching DGHs in Scotland. In addition the hospital is currently the base for provision of surgical, orthopaedic, ophthalmology, paediatric and the majority of maternity services to the population north of the Clyde served by the Vale of Leven Hospital.

The hospital provides a wide range of District General Hospital specialties with excellent support facilities. The hospital has a first class modern radiology department and services. Pathology services for the Clyde division of Greater Glasgow and Clyde (GG&C) are centered on this site. All departments participate in undergraduate teaching and there is an active postgraduate educational programme. The hospital enjoys an enviable reputation for undergraduate teaching and is highly popular with training grades. There is a good medical library service. At the beginning of 2005 Paisley University Nursing School left the RAH campus. Proposals for the use of the vacant space at the heart of the hospital are being sought and are likely to include the further development of ambulatory care, day surgery, endoscopy services and improved educational facilities.

2. The Medical Unit

The Medical Unit at the Royal Alexandra Hospital has 209 beds. This includes an 18 bedded Coronary Care / Chest Pain Unit, a 30 bedded medical receiving ward and Medical Assessment Unit.. The medical unit provides an acute service in general medicine with clinics in general medicine, diabetes and endocrinology, gastroenterology, cardiology, asthma, respiratory medicine and haematology.

The Vale of Leven Hospital medical unit includes a Medical Assessment Unit, 12 bed receiving unit with provision for monitored beds and a further 27 general medical beds.

There are Specialist Nurses in Diabetes, Respiratory Medicine, Cardiac Rehabilitation, Cardiac Failure, Haemato-Oncology, D.V.T and Chest Pain management and Inflammatory Bowel Disease across Clyde.

6. Gastroenterology Services

1. There are thirty nine inpatient beds provided across wards 27 and 14 at RAH for Gastroenterology.

2. Staffing for Gastroenterology services at Royal Alexandra Hospital will be five Consultant Physicians.

3. Daycase and outpatient facilities include: Two endoscopy rooms in the Day Surgery unit at RAH and one room available at VOL. ERCP is carried out by two Physicians and one Surgeon.

4. Two Consultants provide outpatient and endoscopy sessions at Vale of Leven Hospital.

5. Nurse specialists in Colorectal disease, Alcohol Liaison and Hepatitis C are already in post. An anorectal and oesophageal manometry service is provided by Surgical Nurse Specialists.

6.5 There is an alcohol liaison service .

7. Acute Medical Services

The team of Consultant Physicians at the Royal Alexandra Hospital have responsibility for acute medical services at both the Royal Alexandra Hospital and the Vale of Leven Hospital.

7.1 Acute Receiving Ward

GP referred medical patients are triaged from the Emergency Department to the Medical Assessment Unit at Royal Alexandra Hospital Monday to Friday 10am to 10pm. All patients are reviewed by a senior doctor of which just under 50% are discharged home. Patients requiring admission can be admitted to the relevant specialty bed direct from MAU.

Acute medical admissions are admitted to the Acute Medical Unit (ward 2) at the RAH, which is staffed by one of the consultants on a rotating ‘Physician of the Week’ basis Monday morning to Friday lunchtime and an Acute Care Physician. Patients are then triaged to the appropriate specialty wards if not for early discharge.

Acute medical admissions at the VOL are admitted to ward 3, staffed by a third consultant on a rotating ‘Physician of the Week’ basis Monday morning through to Friday pm. The Physician of the week will also lead inpatient ward rounds at the Vale of Leven Hospital during this week.

Weekend cover is provided by two Consultant Physicians with joint morning ward rounds at the RAH and afternoon ward rounds at both the RAH and VOL.

7.2 Coronary Care Unit

Patients with acute myocardial infarction, acute coronary syndromes, arrhythmias or congestive cardiac failure at the RAH are admitted to the 18 bedded coronary care unit, staffed during the day by one of the consultant cardiologists on a rotational basis, supported by junior medical staff from the cardiology ward.

7.3 Medical High Dependency Unit (HDU)

The RAH has three medical HDU beds at present within the combined 12 bedded HDU, with scope to increase to four or more medical HDU beds if required.

7.4 Junior Doctors out-of-hours

A Hospital @ Night service started at the RAH in August 2006. This is part of a wider work assessing the impact of modernizing Medical Careers and implementing plans to support the delivery of high quality sustainable services.

Junior medical support at VOL is provided by GPST1 and GPs supported by the roll out of HAN Practitioner service.

8. Medicine for the Elderly Service

Medicine for the Elderly is part of the Rehabilitation and Assessment Directorate, but many of its activities and adult medicine are combined. In particular, out-of-hours cover is provided by juniors from Medicine for the Elderly and adult medicine and both department share educational meetings.

The Medicine for the Elderly Consultants visit the acute receiving ward at the RAH on a daily basis and arrange transfer to the Medicine for the Elderly Unit of those appropriate patients. A Stroke Unit (Ward 4) taking all age groups is led by Dr L Erwin.

The service at the Vale of Leven will support post-admission care of appropriate patients with 37 rehabilitation beds which will also provide near to home rehabilitation for patients admitted initially to the RAH.

9. Management Arrangements

Emergency Care and Medical Services Directorate – Clyde Sector, consists of Emergency Medicine and General Medicine. The consultant appointed will be expected to participate fully in the work of the Directorate.

The Clinical Director for Medicine across Clyde is Prof Martin McIntyre, based at RAH. There are two Lead Clinicians for Medicine across Clyde: Dr Stuart Hood at RAH/VOL and Dr Chris Jones at Inverclyde Royal Hospital.

10.3 On call duties

The successful candidate will share the four allocated ‘Gastroenterology’ slots for receiving and oncall with the team of Consultants.

10.4 Support staff

The unit will try and support the candidate in development of any specialty interest or technique in which they wish to participate.

11. Postgraduate and Undergraduate Training

The unit has a postgraduate programme which includes a weekly unit meeting, monthly journal club and SHO tutorial sessions. There are weekly hospital postgraduate meetings during term time. Mr Andrew Renwick is the postgraduate tutor.

The Medical Unit teaches a number of medical students from Glasgow University. As part of the new curriculum teaching of years one to five is becoming established.

| |New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on |

|SUPERANNUATION |You have the option to join the NHS Superannuation Scheme, to participate in the State Earnings Related |

| |Pension Scheme or to take out a Personal Pension. Employee’s contributions to the NHS Scheme are Tiered |

| |based on your earnings and the employers contribution equates to 13.5 % of salary. Employees in the NHS |

| |Scheme are “Contracted-out” of the State Earnings Related Pension Scheme and pay a lower rate of National |

| |Insurance contributions. Employees who choose to participate in the State Earnings Related Pension Scheme|

| |pay the higher rate of National Insurance contribution. A Stakeholder Pension is also available. A |

| |Personal Pension is a private arrangement agreed with the pension provider that will be an organisation |

| |such as a Bank, Building Society or Insurance Company. |

|REMOVAL EXPENSES |Assistance with removal and associated expenses may be given and would be discussed and agreed prior to |
