HISTORY OF THE HOSPITAL

Northern Hospital of Surry County is a general

short-term hospital which opened in April 1957. The

hospital was constructed under the Hill-Burton Act at a

cost exceeding $1 ,750,000. The hospital district contributed through tax funds $500,000 toward the construction cost, and tax funds make up about 5 percent

of the annual operating budget.

Consisting of 100 beds and 25 bassinets, the hospital has the most modern equipment available. It is

staffed with over 225 employees and students in order

to provide the best possible patient service around-theclock. 7 days a week. The Martin Memorial Nursing

Program and the School of Medical Technology are

operated in conjunction with the hospital.

Medical and dental services are provided by a staff

of over 30 doctors. The board of trustees delegates

authority to the administrator for operation of the

hospital.

The hospital is licensed by the North Carolina Medical Care Commission and approved by the Department

of Health , Education , and Welfare for treatment of

Medicare and Medicaid patients.

In March 1974, the Ambulatory Care Center addition

to the hospital was completed . This furnishes expanded

outpatient and diagnostic services to patients.

Welcome to Northern Hospital of Surry County! Our

entire staff is dedicated to providing you excellence in

patient care in an efficient, yet personal , sympathetic,

and dignified manner. These are the principles which

guide us in our efforts to relieve you of your discomfort

and to help you recover from your illness.

Within its capabilities the hospital staff will respond

to your request for service consistent with the policies

of the hospital. Such service may include evaluation,

treatment, and/or referral according to your physician 's

assessment of your case.

All hospital personnel are obliged to respect your

privacy. Any examinations or conversations with your

physician and the hospital staff will be kept in strict

confidence and released only with your permission. You

will come into contact with health personnel in training,

but this will not violate your confidence or privacy.

Bring information about past illnesses, hospitalizations, medications, and other matters relating to your

health.

Be open and honest with us about instructions you

receive concerning your health ; that is, let us know

immediately if you do not understand them or if you feel

you cannot follow them.

Do follow the policies adopted by the hospital , which

are developed in your own best interest.

Don 't hesitate to ask questions you may have concerning your hospital bill as well as providing information necessary for insurance processing and paying any

amount which becomes the patient's personal liability.

Your welfare is the personal responsibility of

everyone here. It is our objective to provide you with

such comforts as well-prepared and promptly served

food ; a pleasant and restful atmosphere ; respect for

personal privacy and dignity ; a neat and clean room ;

quiet and uncluttered halls ; and prompt response to

your request for assistance.

If you feel that this hospital has not met these standards, we would like the opportunity to take corrective

action. We suggest that you discuss the matter first with

your physician or nurse. If this approach is not feasible

or proves unsuccessful, please call or write the hospital

administrator's office ; your problem will receive prompt

and personal attention .

It is our sincere hope that your stay at Northern

we will be instrumental in your prompt recovery .

ADMISSION TO THE HOSPITAL

Your admission to the hospital was requested by

your physician who is a member of the hospital medical

staff. At the time you entered the hospital , you were

interviewed by admitting personnel. This information is

necessary and we appreciate your cooperation . In some

cases, your doctor will use a preadmission form

furnished by the hospital to expedite your admission

procedure. Be sure to bring information about your

hospitalization insurance.

Unless you are an emergency patient, you should

arrive at our admitting office between 11:00 a.m. and

3:00 p.m. This is especially important for patients

scheduled for surgery the next morning . Of course,

emergency patients are admitted any hour of the day or

night.

Permits for treatment and/or operations must be

signed by each patient or by next of kin. Permits for

minors must be signed by parents or guardians.

YOUR HOSPITAL BILL

Your hospital account is payable upon discharge

from the hospital. You will be given an itemized copy of

your account. If you stay longer than 1 week, your account is payable every 2 weeks while hospitalized .

Ctedit will be given on assigned insurance subject to

the insurance company 's approval and payment. On

many group insura nce policies, it is necessary to obtain

insurance claim forms from your employer and bring

them to the hospital business office by the time of

discharge.

Your hospitalization coverage is a contract between

you and your insurance company, and we will cooperate to the fullest in expediting your claim . You, of

course, are ultimately responsible for your account.

FINANCIAL COUNSELOR

Patients who feel their hospital bill is becoming

more of a financial burden than expected should

contact the patient accounts manager in the hospital

business office at once. In some cases, installment

payments or other relief can be arranged if an investigation indicates that there is financial need .

VISITOR CODE

If these visiting practices are followed , your hospital

stay could be shortened . Too many visitors staying too

long can easily t ire you and prevent the hospital staff

from giving the necessary treatments. Please help us

you r family and friends. Your doctor has approved this

vis itor code as important to your health and recovery.

Intensive Care Visiting Hours

10 :00 a.m. to 10 :10 a.m.

1 :00 p.m. to 1 :10 p.m.

4 :00 p.m. to 4 :10 p.m.

8:00 p.m. to 8 :10 p.m.

Only immediate family-one at a time.

? Doctor's written permission necessary for person to

stay overnight with a patient.

? Only two visitors per pat1ent.

? Visits limited to 10 to 15 minutes.

? Children under 12 years of age not allowed to visit

patient's room .

? No food taken to upper floors for consumption by

visitors.

? Visitor smoking restricted in certain circumstances.
