Extracorporeal Membrane Oxygenation (ECMO)

Oklahoma Heart Institute (OHI) is the first hospital in northeast Oklahoma to offer ECMO for adult patients who are extremely sick due to heart and/or lung failure. ECMO helps alleviate the stress on these vital organs in order for them to have a chance to rest and recover until they are able to properly function again.

OHI has the capability of placing patients on ECMO remotely and then transporting them back to our hospital to continue this lifesaving therapy.

Depending on the condition of the patient, they may stay on ECMO anywhere from days to several weeks. During this time, our medical team is able to treat them with a variety of procedures, medicines or give them adequate time to recover.

The following outlines patient parameters we look for when accepting ECMO referrals:

INDICATIONS ?Refractory hypoxic respiratory failure ?Refractory hypercapnic respiratory failure ?COVID 19, influenza ? ARDS ?Pulmonary embolism ?Status asthmaticus

?Cardiogenic shock ?Acute myocardial infarction ?Postcardiotomy syndrome ?Persistent unstable arrhythmias ?Drug overdose ?Septic/circulatory shock ? Trauma ?Amniotic fluid embolus ?Hypothermia with temperature dependent dysrhythmia ?Post cardiotomy cardiogenic shock

RELATIVE CONTRAINDICATIONS ?On high ventilator settings more than 7 days ?Difficult intravascular assess ?Over 75 years old ? Obesity ?Chronic organ dysfunction

ABSOLUTE CONTRAINDICATIONS ?Severe multiorgan failure ?Underlying disease with poor prognosis ?Unrecoverable heart and not VAD or transplant candidate ?Status post CPR without known neurologic recovery ? Asystole ?Aortic dissection ?Refusal to accept blood products ?Absolute contraindication to anticoagulation ?Recent intracranial bleed
