Dr. Fleming obtained his B.S. in Physics/Computer

Science from Alabama A&M University in Huntsville,

AL. His medical doctorate was completed at Michigan

State University College of Osteopathic Medicine in East

Lansing, MI. He completed his internship in general

surgery at the Naval Medical Center in Portsmouth,

VA and his orthopaedic surgery residency at Michigan

State University, Mount Clemens Regional Medical

Center, in Mount Clemens, MI. Dr. Fleming went

on to complete a fellowship in orthopaedic trauma

from the University of Minnesota, Hennepin County

Medical Center, Minneapolis, MN and Regions Hospital

in St. Paul, MN. Dr. Fleming is a Washington State

Department of Labor and Industries approved provider.

He is able to address all musculoskeletal concerns in

the IME setting.

Dr. Faloon obtained a B.S. in Biology, Magna Cum

Laude, from Allegheny College in Meadville, PA. He

completed his medical doctorate from State University

of New York, Upstate Medical Center, in Syracuse, NY.

He also completed his internal medicine internship

from State University of New York and his general

surgery internship from Mercy Hospital, Pittsburgh,

PA. Dr. Faloon completed his residency in orthopaedic

rehabilitation at Rancho Los Amigos Hospital in

Downey, CA and his othopaedic surgery residency

at the University of Pittsburgh, Presbyterian Hospital,

Pittsburgh, PA. He went on to complete a fellowship

in sports medicine at Temple University Hospital,

Philadelphia, PA, a foot and ankle fellowship from the

Cleveland Clinic Foundation, Cleveland, OH and a total

joint fellowship from Harrington Arthritis Foundation

in Phoenix, AZ. Dr. Faloon is a Washington State

Department of Labor and Industries approved provider

and he is able to address all musculoskeletal concerns

in the IME setting.

KYLE L. PALMER, M.D.

MICHAEL SCARCELLA, M.D.

Dr. Palmer attended the University of California,

Irvine, CA and California State University, Long

Beach, CA where he obtained his B.A. degree. He

obtained his medical doctorate from the Medical

College of Virginia, Richmond, VA. Dr. Palmer

completed both his internship in general surgery

and his residency in orthopaedic surgery at the

University of Tennessee, Memphis, TN. Dr. Palmer

is able to address all musculoskeletal concerns in

the IME setting.

Dr. Scarcella obtained a B.S. in Biology

from Baldwin-Wallace University in Berea, OH. His

medical doctorate was completed at Northeastern

Ohio Medical University in Rootstown, OH.

He completed his internship and orthopaedic

residency at the Cleveland Clinic Foundation in

Cleveland, OH. Dr. Scarcella is a Washington State

Department of Labor and Industries approved

provider and is able to address all musculoskeletal

concerns in the IME setting. He is in active practice

in the Puget Sound area.

Dr. Palmer is available for IME scheduling in Boise, ID.

Dr. Yerby obtained a B.S. from the University of New Hampshire in Durham,

NH and attended Mount Sinai School of Medicine in New York, NY. His medical

doctorate was obtained from the University of Vermont in Burlington, VT and his

neurology residency at Oregon Health Sciences University, Portland, OR. Dr. Yerby

went on to obtain his Masters in Public Health from the University of Washington in

Seattle, WA. Dr. Yerby is an Oregon WC approved provider.

Dr. Hackett attended Holy Cross College in Worcester, MA and obtained his medical

doctorate, internship and residency from Georgetown University, Washington, D.C.
