ADOPTION CONTRACTDutch Country Animal Rescue rescues, rehabilitates, and re-homes abused and unwanted animals from puppy mills, high-kill shelters, and other situations where our help is most needed. It is our desire to ensure the safety and well-being of all animals we place. We recognize that sometimes animals placed in even the best homes may not work out and in this case, we believe it is in the best interest of the animal to be re-homed.We cannot guarantee the health or behavior of any animal that we place, nor can we reimburse for any medical expenses incurred because of an unknown issue. However, we have a full-disclosure policy and provide all known information about the animal prior to signing this agreement. Because Dutch Country Animal Rescue wants to ensure our animals are in a loving environment where they are wanted, we have instituted the following policies: We will accept the return of any animal placed by our organization at any time. If the animal is returned within 10 days, Adopter will be refunded the adoption fee minus a $100 fee. Adopter is responsible for returning the animal to the organization. If the organization has to arrange to pick up the animal, Adopter will be charged reasonable transport costs to cover gas and tolls.If there is report of abuse of the animal, Dutch Country Animal Rescue reserves the right to terminate this agreement and collect the animal. If the adopter cannot keep the animal for any reason, the dog must be returned to our organization so that our organization can ensure the animal is rehomed to a safe environment. The animal cannot be turned in to a shelter, sold, or given to a friend or family member. If a friend or family member would like to take the animal, they must adopt the animal through our organization.In return, we ask that the adopter of this animal agree to the following:To return the animal to our organization in the event the adopter can no longer provide care for the animal.Have provided a family member or friend with instructions to return the dog to our organization in the event the adopter is no longer able to care for the animal and unable to ensure the safe return of the animal on their own.To provide an indoor environment for the animal to live, which includes not leaving the animal outside while not at home, during the night, during inclement weather, or for extensive periods of time. To provide a comfortable indoor environment for the animal to live, which includes not leaving the animal in a garage, unfinished basement, or outbuilding (e.g. shed or barn), nor chaining the dog to any object. We discourage the crating of animals since many of the animals we place have spent most or all of their life in a cage and our desire is for them to live outside of a cage for the remainder of their life. To provide adequate food and water daily. To provide regular medical care by a qualified veterinarian, including a yearly exam, and required care as determined by the vet. We encourage all adopters to bring their new pet to a qualified vet within 2 weeks of adoption to ensure no unknown medical conditions exist that could pose a risk to other pets in the house or to the adopted animal. Not to sell or give or allow the adoption of the animal to another party or abandon the animal in a shelter or at any other location. Not to intentionally hurt the animal physically or emotionally in any way.The Adopter agrees to pay a donation to Dutch Country Animal Rescue. The Adopter understands that this donation is not a fee or sale price, but rather a donation to help offset the costs of rescuing this and/or other animals. This donation enables the organization to continue to rescue additional animals in need. The required donation is set as follows, unless otherwise specified by an authorized member of Dutch Country Animal Rescue: $500 for dogs under 1 year old, $425 for dogs 1 to 7 years old, $325 for dogs 8 years or older.This agreement is between Dutch Country Animal Rescue, a Pennsylvania non-profit organization, and the adopter ( ), according to the terms of contract set above.This agreement is for the adoption of a rescue, as specified below:Name of Animal:Breed: Gender: Age: Color:Microchip ID:Rescue Number:Additional information:Adopter’s Name: Address:Home Phone: Cell Phone:Email Address: Driver's License Number: I agree to the full set of terms outlined in this Agreement.
