WieczorekDate: 10.2019Department: HVT Service LineHR Approval: nnlDate: 10.2019JOB SUMMARYThe Clinical Program Coordinator Heart, Vascular and Thoracic (CPC) is responsible for coordination of efficient, safe and timely program development in conjunction with triad MD partnership. The programs include all UW Health and affiliated sites providing Heart and Vascular services. The Coordinator actively participates in the development and implementation of goals, strategies, and tactics to support the continued growth and success of the UW HVT Service Line Programs. The Clinical Program Coordinator works in collaboration with a multidisciplinary team of physicians, advance practice providers, registered nurses, nutritionists, medical assistants and appointment schedulers to plan, direct and evaluate the care provided to HVT Service Line patients, and as a result must possess expertise in clinical care standards and workflows. The CPC relies on evidence-based practice standards to assure consistent high-quality care and a positive patient and family experience across the continuum of care. This involves coordination of meetings, data collection and patient and staff education and care coordination with inpatient and outpatient teams at The American Center, University Hospital, Swedish American Health System, and affiliated sites. The CPC maintains a database for quality metrics and identifies and implements process improvement initiatives. The CPC communicates effectively with all faculty and staff at all sites of service using tact, discretion and diplomacy, is skilled in problem solving techniques and team development and provides feedback to both staff and administration.MAJOR RESPONSIBILITIESClinical Program CoordinationServes as primary point of contact to triads and promptly responds to requests for program information.Maintains relevant policies and procedures and coordinates patient education, outcomes data collection, quality improvement efforts, and education of relevant staff in the various aspects of the HVT Service Line.Serves as a liaison with the medical staff to identify issues of mutual concern, solve problems, review care plans and optimize clinical operations.Meeting planning and coordination for triad team and stakeholders.Enforces adherence to hospital and department policy and procedures.Collaboration with HVT Service Program Director, clinic management, and multidisciplinary care team to support best practice and prompt attention to support needed.As directed, participates in department-level work groups and committees to address operational, quality, and safety initiatives.Program Growth and Strategic PlanningCollaborates with administration, providers, and staff in program and business planning efforts to develop and grow programs within the HVT Service Line.Coordinates patient and referring provider outreach activities in support of HVT clinical program growth.Regularly reviews clinical, operational, and financial data to monitor program success and identify opportunities for municates with and engages program stakeholders within and outside of UW Health.Develops and implements action plans to enhance program growth and access as well as provider productivity.Education/OutreachCreates an environment in which learning is supported and valued.Interprets and supports the philosophy, objectives, and policies and procedures that guide personnel, patients and the public.Participates in multidisciplinary meetings.Meets personnel continuing education and developmental needs.Coordinates appropriate written patient education materials.Serves as a resource to staff in other clinics as appropriate.Supports and/or participates in research programs approved by the Hospital and the University.Supports and/or participates in community education and outreach programs.Supports and/or participates in clinical research activities approved by the hospital.ALL DUTIES AND REQUIREMENTS MUST BE PERFORMED CONSISTENT WITH THE UW HEALTH PERFORMANCE STANDARDS.JOB REQUIREMENTSEducationMinimumBachelor’s degree in NursingPreferredMaster’s degree in business administration, management, public health, or related fieldWork ExperienceMinimumThree (3) years of work experience in a health care setting Experience leading programs and/or project with teamsPreferredExperience in Cardiology and Surgery program development.
