About this document: hospital passportFor some of us, it has become important to make plans for if we become very ill and need to be admitted to hospital. With that in mind, I’ve prepared a hospital passport. This relates to mine and my family’s needs - but if it is useful to you, please adapt to fit your circumstances.There are general disability healthcare/hospital passport templates available from organisation such as National Autistic Society; many of these can be found on the internet. This is a reduced version that aims to communicate only the absolute essentials for a disabled person who has contracted the virus and is sufficiently ill for hospital admission. You might choose to underplay aspects of impairment that it are not critical for healthcare practitioners to know about.(This template is for emergency admission/triaging. Once admitted to a regular ward or in the case of other treatment or admission, you could use a standard healthcare/hospital passport or support plan. WECIL has a free support plan toolkit at: )Please take a breath before the next bit…NICE (National Institute for Healthcare and Excellence) has issued ‘rapid guidelines’ to clinicians in relation to managing coronavirus (COVID-19). These include guidelines for clinical decisions in allocating scarce resources during a health emergency. These guidelines use a ‘clinical frailty scale’ which poses a direct threat to some disabled people’s wellbeing. Whilst legal challenges are underway, and (as of 23/3/20) guidelines have been amended as not applying to healthy disabled people under the age of 65, we have also been considering measures that can be taken by us as individuals to increase our personal protection during this health crisis, especially for those who are ill or aged over 65.In view of the NICE guidelines, we suggest that you consider:Underplaying details of your impairment and its effects (whilst also being honest about relevant or critical details).Using the document to convey your vitality, social contribution and relationships.With this in mind, the passport includes the addition of a photograph and brief information that establishes 'reasons for living', as well as an anti-DNR message.I suggest that four copies (minimum) are made available: one to paramedics if travelling by ambulance; one tucked clearly into clothing for admissions/ward staff; one left with family member; and one with an advocate who, where possible, does not live in your household.This is a horrible document to put together for any of us and, of course, I hope no one will ever need to make use of it – but if we do, it might increase our odds of receiving the treatment we ments welcome in The Bunker on how it needs amending, or how you are adapting it to your own situation.Wishing you well.Note that Bunker Admin are not medically trained and are issuing these guidelines based on our best interpretation of the risks of this health emergency to disabled people.
