How do patients know when they are receiving the "best" asthma care?

How do care process teams (physiciansnurses-respiratory therapists-care coordinators) pinpoint the steps that need to be improved for better outcomes?

Tools that help us measure or quantify healthcare processes, outcomes, patient perceptions, and organizational structures

Measures show where systems are breaking down and where they are succeeding to help patients get and stay well.

Related to one or more quality goals for health care: effective, safe, efficient, patient-centered, equitable, and timely care.

Types of Quality Measures

Reflect the conditions in which providers care for patients.

Examples:

Proportion of specialist to other physicians at a hospital Transition plan from pediatric to adult care for patients with

chronic conditions

Show whether steps, proven to benefit patients, are followed correctly.

They measure whether an action was completed. Examples:

Percent of asthma patients that received an asthma management plan

Time from arrival to administration of an antibiotic

Do not take stock the processes, but of the actual results of care.

They are generally the most relevant measures for patients and the measures that providers most want to change.

Examples:

Rate of readmissions for asthma-related issue Number of school/work days missed Patient Satisfaction Mortality

Assess whether changes implemented have caused unintended consequences

Example:

Goal: Decrease length of stay for asthma related hospitalizations

Balance Measure: Assess whether the decreased length of stay has affected the rate of readmissions for those patients

One approach to measuring quality is the use of a scorecard, which summarizes a critical set of quality measures based on an organization's strategic plan or collaborative's objectives

Importance Does this measure represent a priority or high impact aspect of healthcare? Are improvements in this measure important to the health outcomes of your target population?

Scientific Acceptability Will the metric produce consistent and credible results about the quality of care when implemented?

Usability Does this measure provide information that is actionable (can the measure be used to improve quality)?

Feasibility Readily available data, retrievable without undue burden and can be implemented for performance measurement.
